      • -Hand wash sink in Dragonfly room is located outside the bathroom door. There is no hand wash sink inside the bathroom and children/staff are handling the door with unwashed hands. Please install a hand wash sink/remove the door to this bathroom. Please consult licensing prior to removing door to determine if this is appropriate for this age group.

      • ***Back door entry is missing trim around the doorframe exposing rough edges and raw unfinished material. Have the back entry around the door finished so that its in good repair, smooth, easily cleanable surfaces and impervious to moisture.-Hand wash sink in Dragonfly room is located outside the bathroom door. There is no hand wash sink inside the bathroom and children/staff are handling the door with unwashed hands. Please install a hand wash sink/remove the door to this bathroom. Please consult licensing prior to removing door to determine if this is appropriate for this age group.

      • Back door entryway has weatherstripping in disrepair providing large gaps underneath the door. Have the weatherstripping replaced so that there are no gaps between the door and the floor.**Weatherstripping has improved but there are still gaps.

      • ** Large area needed to use to access newly constructed rooms has unfinished flooring that is not impervious to moisture and easily cleanable, along with unfinished kitchen development. Have the area completed so that all accessible areas are easily cleanable, impervious to moisture. Construction site open onto child space would not be considered appropriate or acceptable.**Diaper stations were noticed to be in disrepair requiring repair/ replacement. Butterfly room station back cover need to be redone as damaged. Have the back cover addressed so that it is in good repair, impervious to moisture and easily cleanable. Baby room diaper station top edge is in disrepair. Have the top edge repaired of the diaper station or it replaced so that it is in good repair smooth easily cleanable and impervious to moisture.**Butterfly room there was a shelf that the screw was sticking out past the surface. Have the screw properly addressed so that its flush with the surface.*** Ladybug room there is unfinished section behind the fridge and counterspace exposing unfinished drywall and raw wood. Have this area finished so that all surfaces are smooth, easily cleanable, and impervious to moisture.***Dragonfly room bathroom wall is missing tiles in area and has holes in the wall where handwash sink use to be exposing unfinished areas. Have the walls in the bathroom repaired and finished so that they are in good repair, easily cleanable, and impervious to moisture.***Back door entry is missing trim around the doorframe exposing rough edges and raw unfinished material. Have the back entry around the door finished so that its in good repair, smooth, easily cleanable surfaces and impervious to moisture.***Middle back room along the south wall observed coat hook/ rack is unfinished wood. Have finished. -Handwash sink in Dragonfly room located in carpeted area would be considered not appropriate due to the carpet trapping moisture and it being increased risk of mold growth.

    • 08. Is this facility properly constructed (Non-Critical)?
      • *Observed that new/ future space to be used for Ladybug still requires doorframes in the area to be trimmed out the unfinished edges of drywall around the doorframe. Operator informed that they are to be trimmed. *Observed that washroom in back area next to ladybugs that was to be just for staff but will be used by the children does have some gaps between the wall covering material. -Have the gaps addressed and finished so that there are no gaps and the surface is smooth and easily cleanable.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • *Facility has few areas within the facility that they will be touching up the paint on the walls. Indicating that they are ensure constant maintenance is being done. Ensure that patched surfaces are finished soon so the surfaces is smooth and easily cleanable.*Observed that in the back area next to the washroom entrance that there are two bolt end sticking out of the wall surface. Advised that these bolts be addressed or covered so they do not pose a hazard for someone to crash/ bump into.

    • 06. Are proper protocols being followed in regards to client care?
      • *Chlorine test strips were not able to be located. Have test strips located, and available for use and confirmation. *Diapering procedure is still to be posted. -Chlorine based disinfectant is being used for the changing tables. Bleach solution was found to have chlorine concentration @ ~200ppm. Facility follows pre-determined amount of bleach to achieve the set concentration. Dedicated diapering areas with changing table and surface that can be easily cleaned and disinfected between uses.

      • *Few spots around the 1/2 doors framing and trim that require paint. Staff to paint and correct today. Have the surface finished so that it is smooth, easily cleanable, and impervious to moisture.

    • 08. Is this facility properly constructed (Non-Critical)?
      • Multiple areas are still under construction that will need to ensure that have been addressed. 1. multiple areas with missing baseboards (1st bathroom, butterfly room, middle space (bumble bee & dragonfly), caterpillars, and ladybugs room. 2. Door trim is missing in butterfly, 1/2 door to be installed in middle area.3. Outlet covers are still to be installed for the facility, along with the replacing in places of ceiling tiles, and access panel. 4. Bathroom exhaust fan to be installed in the 1st bathroom. 5. Back area to have bathroom renovated, and area for 2 compartment sink for cleaning. Future of this back area will be re-addressed at a later date and time.

      • Some of the handwash sinks are setup and equipped. Sinks to be setup include caterpillar, ladybug, and back kitchen area all of which dont currently have their sinks setup.
