      • Observation (CORRECTED DURING INSPECTION): Cold potentially hazardous food (milk and cream) stored in undercounter cooler near the pop dispensing machine registered an internal temperature > 4 deg C (8.1 - 9.2 deg C). Milk and cream have been stored in the unit for an undetermined amount of time.

      • Continue to work with your pest control company for monitoring and baiting.
      • Keep up with your sanitation schedule and ensure cooking area does not build up grease.
      • Ensure to not store any items underneath the cooking area and underneath prep sink and dishwashing sinks. Cleaning schedule to be made available on-site for inspection.

      • Cooking area (underneath flat grill and other cooking elements) as well as the floor (especially underneath the fryers) still have grease/oil and debris build up.
      • Corrective Action(s):
      • Cooking area must be thoroughly cleaned. Remove grease and debris and sanitize.
      • Sanitation is an integral part of addressing rodent issue in the facility. Grease and food debris are attractants to rodents. Management must demonstrate that they are doing everything to address the pest issues by actively working with the pest control company as well as keeping a high level of sanitation in the premises.

      • 1. Sanitizer (quats) in spray bottles have less than 200 ppm solution.
      • 2. Glass ware washer had not iodine solution. Sanitizer container was empty.
      • Corrective Action(s):
      • 1. Re-fill containers with fresh solution daily and check dispenser that
      • it is working properly to ensure the appropriate concentration is used.
      • 2. Ensure to check sanitizer concentration on a regular basis as well as
      • checking to ensure all chemicals are full.

      • Observation:
      • Just underneath the cooking elements (flat grill, stove, etc.): there were grease
      • build up and debris of foods noted and also some mice droppings were observed.
      • Fume hood grill vents have build up of grease as well. Floor underneath the cooking
      • area has grease and debris build up.
      • Corrective Action(s):
      • Clean all equipment and underneath cooking elements thoroughly to
      • remove grease and debris build up. Sanitizer thoroughly with bleach after cleaning.
      • A good, thorough sanitation helps in
      • preventing pest issues.

      • Observation: There is still some mice activity that is being found by per pest control company and management. There was sighting of mouse by customer at time of visit as they mentioned it to the manager.
      • Corrective Action(s): The corner booth area beside the ATM machine have signs of pests activity. Droppings were found in the ATM machine and cleaned thoroughly. A bait trap is placed inside machine. This area will be monitored closely and hole into the machine to be plugged with steel wool until it can be permanently closed off. Pest control measures actively in place to make the premises free of pests.

Include observations and comments (i.e. cleaned and sanitized and when new droppings are seen). Work with your new pest control company to find measures to eliminate pests inside the premises.

      • Corrective Action(s): Ensure all spray bottles labelled sanitizer have proper sanitizer concentration.
      • Empty and fill these two spray bottles with fresh sanitizer. Use test strip to check for concentration and al so check the sanitizer dispenser that it is working properly.
