- Infant room: Diapering pad was stored on top of the toy storage shelf (under the fold down diaper change table). An additional diaper change pad was on the fold down diapering table. Ensure that the diapering change pad is stored out of reach of children and in a dedicated area.

- Children were observed eating afternoon snack as they arrived to the program in the OSC room. Snack was consumed without handwashing observed.Reviewed with staff on site proper handwashing procedure and times for handwashing including before and after eating snack. Children were observed washing hands after finishing snack.

- OSC room: Hand sink in room - indicated to be used for hand washing for staff. No soap or paper towel was available. Ensure all hand sinks have soap and paper towel available.

- OSC washrooms:Girls washroom - storage container was noted to be unsanitary with dirt and debris. Ensure all surfaces are cleaned and disinfected regularly.Boys washroom - urinal was removed. Cardboard was in place over areas of the wall that were open, which is not cleanable. Repair the wall to be in good repair, smooth and easy to clean condition.

- Toy washing procedure was discussed with proper procedures provided by staff. Toy bins in several rooms were noted to have dirt and debris in the bottom. Ensure staff are cleaning and disinfecting toys and bins when soiled or weekly.

- Toddler/Preschool washroom: 1. 2 sinks were not operational (first two near the room). 2 additional sinks were operational.Ensure all sinks are operational at all times with proper hot and cold water.2. 1 toilet was not operational.Ensure all plumbing including toilets are operational.

- Mouse droppings were noted behind the dryer/washer area. Several bait boxes were noted in the staff room, laundry area around the door. Professional pest control is required to be contracted to ensure pest control measures including bait are being conducted in a safe manner.

Repair/replace affected components, ensure that adequate supply of hot and cold water is provided in the hand sinks. Updated on December 13, 2023: hot water was provided but the temperature was too hot and measured 51.5C. Please ensure that hot water temperature is below 49C.

- 1. Some staples were loose in the children's area in the Preschool room. Remove loose staples. If staples are used in children's areas, cover them with pieces of tape.2. Some art supplies (paints) labelled to be used for children over 3 were observed in the Toddler room. Ensure that the art supplies are appropriate for the children's age.

- Water stains were observed on the ceiling in the Infant room and OSC room. Investigate for the source of moisture and replace affected tiles.

- Antiseptic wipes observed in the first aid kit expired in December 2017 and November 2019. Provide adequate supplies for the first aid kits.

Updated on November 20, 2023: Some light fixtures were not operational/not operating properly in the Toddler room (3 light fixtures not in operation), Preschool room (2 lights), OSC room (1 fixture in the sink/fridge are), main washroom (1 out of 2 not operational), and Boys' OSC washroom (some bulbs missing, lighting low).

- Leather bench was damaged and peeling in the Preschool room and leather couch was damaged in the OSC room. Repair, replace or remove. Updated on November 20, 2023: Couch was damaged (filling exposed) and stained in the OSC room, the armchair was stained in the OSC room, and the leather bench was repaired with a duct tape in the Preschool room. Repair, replace or remove affected furniture (repair with duct tape can only be used as a temporary measure).

- Hooks used for children clothes storage were too close together (8 inches apart) in the Toddler room and OSC room (some 5 inches apart), and the clothes overlapped in Infant and Toddler rooms. Provide adequate separation (12 inches between the hooks) or place clothes in the large bags, which then placed on the hooks.
