      • The plastic shelves in the walk-in cooler were damaged and deteriorated.-replace any damaged shelves
    • 23. Is the facility maintained in a clean and sanitary condition?
      • 1) The floor and walls under/around the shelves in the walk-in cooler were dirty with dirt, debris, etc.-clean the cooler; move the shelves to thoroughly clean.2) Shelves and cardboard was dirty with spills/debris.-remove all cardboard from shelves. Clean the shelves.
