- Mud used to repair holes in the walls throughout the daycare was unsanded and unfinished, leaving surfaces that are rough and not easily cleanable.Please sand down and finish the indicated areas to ensure the surfaces and smooth and easily cleanable.

- The far corner ceiling tile located above the counter tops by the chalk board were observed to be severely water damaged.Please identify the source of the leak and address the issue. Replace the ceiling tile and monitor the area for leaks.

- The following areas were found to be soiled with stains, loose debris, dirt, food debris, and related contaminants and require a deep clean:- All countertops- The handwash sink- All chairs- All tables- Shelving units storing toy bins (Room 4 and the Gym)- All toy bins- All toys in the facility- Carpets/rugs- The baskets used to store the children's belongings- FloorsPlease deep clean and sanitize all items listed above. Implement a cleaning schedule or check list for staff to follow.

- The water damaged ceiling tile located at the far corner beside the chalk board was observed to be sagging, potentially becoming a falling objects hazard.Please identify and address the leak and replace the ceiling tile.

- The washing and rinsing steps were missing from the three-step toy cleaning method when reviewed with the operator. The operator was spraying the toys with disinfectant and rinsing off the solution without cleaning the toys first.The operator was informed about the three-step method when cleaning and disinfecting toys. Toys must first be cleaned using warm water and detergent, rinsed, and disinfected by submerging the toys in the disinfecting solution (100ppm concentration for chlorine) for two minutes and left out to air dry. Please ensure that all staff members are trained in the three-step method for toy cleaning.
