    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - There was water accumulated on the left side of the tap in the ceramic hand sink in the washroom. The ceramic part was broken/damaged.*Repair or replace the hand washing sink and ensure no water accumulates in the area.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Basement storage area is still full of storage items, unable to access the floor to check for pest activity. -Ensure this area is organized an accessible to identify any pest issues, building issues and to facilitate ease of cleaning. Please stored materials away from the ground and walls for accessibility.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Small toys were observed in the infant room.*The violation was corrected onsite. Staff moved the toys to the other classroom.*Staff will ensure that all toys are safe for the children, age-appropriate, and not a choking hazard. A test using a paper towel roll was demonstrated to the staff.2. Tempera paint (labeled for ages 5+) was observed in the room for children under 5 years old.*Staff discarded the paint. Staff will ensure to follow the manufacturer’s instructions for use and only introduce age-appropriate equipment and play materials.

      • - There was water accumulated on the left side of the tap in the ceramic hand sink in the washroom. The ceramic part was broken/damaged.*Repair or replace the hand washing sink and ensure no water accumulates in the area.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • - Cardboard blocks were being used as building blocks, with cardboard visible/absorbent from the corners. Staff were spraying the blocks with sanitizer to sanitize them. For proper sanitization, the entire equipment/block must be smooth, non-absorbent, and easily cleanable.*The violation was corrected onsite. Staff discarded the cardboard blocks.

The floorboard beside the toilet in the toddler's washroom was separating from the wall. -Please repair and maintain in good repair to prevent water infiltration.
