- *There was a hole on the wall around the plumbing under the dishwashing sink. Repair hole.*Finishes inside the drawers at the coffee/juice machine area are no longer smooth or easy to clean. Ensure surfaces are smooth and easy to clean.

- *Ice cream scoops were found to be stored in a cup of warm water. Discussed storage options for ice cream scoops for in between uses. Best practice is to have a dipper well with a continuous flow of water. Other options include storing scoops in a clean container in the freezer or a fresh scoop for each use. Corrected on site: scoops were placed into a clean container in the freezer.*Scoop was found directly contacting sugar in the sugar dry ingredient bin. Discussed scoop storage options. Ensure one of the following scoop storage practices is followed: utilizing holders, store scoops outside in a clean container and place scoops into dishwashing once they are not required. Corrected scoop was removed and site indicated they will store scoop outside in a clean container.

- *Cleaning and temperature records for August were not observed. Ensure temperature monitoring and cleaning is being done and records are kept as tasks are completed.

- * Bag of onions was observed to be on the floor. Ensure food is always protected from contamination. Immediately remove food from the floor.

- *Thermomether was not observed in the chest freezer in the basement dry storage area. Ensure a thermometer is available and temperature monitoring is done.

- *Weather stripping was observed to be in disrepair. Gaps between floor and door were observed. Ensure wheather stripping is immediately fixed/addressed to ensure no vermin entry.*Fruit flies were observed in the dishwashing area. Ensure action is being taken immediately to prevent fruit fries (i.e., harborage and prevention)

- *Dented tomato sauce Heintz can was observed. Ensure all cans are in good repair to prevent breaking the seal and create an opportunity for bacteria growth. This was immediately corrected by the operator. This was corrected oniste.

- *Mould like appearance was observed in the walk-in cooler on the ceiling. Please contract a qualified consultant to fully investigate & assess the area(s), including hidden cavities & surfaces, for signs of water damage & mould. The consultant shall ensure & document any mould remediation work completed. The extent of mould growth and remediation work involved may require partial or full kitchen closure. Contact EPH and IPC to consult based of extend and alternatives for operation. Have HVAC system in the walk in cooler assessed to determine if the moisture/humidity levels are excessive and/or if system is meeting operational requirements.Please ensure that construction or remediation does not occur until a qualified consultant completes a risk assessment to determine the scope of work required. Complete construction activities in accordance with CSA Z317.13:22. Ensure that walls finished so they are smooth and easy to clean. Walls in areas of high moisture (dishwashing area) should be smooth, easy to clean, and water resistant/impervious to moisture to prevent future damage.

- *Basement dry storage floor appeared to be stained. The operator advised that a flood occurred in the recent past. A moisture meter was used to measure water activity in several areas. Some areas were observed to have some water activity. Please contract a qualified consultant to fully investigate & assess the area(s), including hidden cavities & surfaces, for signs of water damage & mould. The consultant shall ensure & document any remediation work completed. Investigate and ensure that notification/ e-mail is sent to AHS- EPH if flood incidents occur.

- *Dust and debris were noted in several areas. Areas were shown to the operator during inspection (i.e. Walls, ceiling tiles about the ventilation hood, hard to reach areas under the mechanical dishwasher)
