- Observation (CORRECTED DURING INSPECTION): A mouldy mini cheesecake was found inside the small fridge at kitchen
- Corrective Action(s): Do a thorough check of all the fridges/coolers to ensure there is no mould on food products. Operator discarded the mini cheesecake

- Observation (CORRECTED DURING INSPECTION): Ambient temperature of the large fridge inside the kitchen is 6C. Ambient temperature of small fridge inside the kitchen is 7C. Internal temperature of eggs from small fridge is 13.7C. Temperature logs for small fridge over past few days were -10C.
- Corrective Action(s): Ensure the temperature of all fridges are 4C and below. Cold food must be stored at 4C or less to prevent the rapid growth of bacteria and/or formation of toxins that could lead to foodborne illness
- Operator voluntarily discarded 15 eggs and breakfast sandwich. Bowl of poke with no client's name on it was also discarded by operator. After operator adjusted temperature settings of the large fridge, the temperature is 4C.
- Do not use the small fridge to store potentially hazardous foods until it has an ambient temperauture of 4C and below. Operator said that they plan on getting the small fridge replaced instead of getting it repaired.

- Observation: Repeat Violation: Accel 0.5% Hydrogen Peroxide was used to clean and sanitize food contact surfaces. This was noted in last inspection report (September 2021).
- Corrective Action(s): Do NOT use Accel 0.5% Hydrogen Peroxide to clean and sanitize food contact surfaces as it is not a food grade sanitizer. Accel 0.5% hydrogen peroxide may be used to disinfect other surfaces that are not in the kitchen. Use 100-200 ppm bleach solution or QUATS sanitizer to sanitize food contact surfaces. Instructions to prepare 100-200 ppm bleach solution provided to staff.
