      • Missing flooring piece around door was observed. Please ensure the floor is in good repair. All doors observed to have holes at the top of the door. Ensure all areas are easily cleanable and impervious to moisture.

      • Safety belts for chairs were observed to be missing. Ensure clients are protected and safe at all times.

      • Sanitizer solution bottle was observed to not be labeled. Please ensure bottles are labeled to show contents. This was corrected during the visit. Bingo dabbers without AP/ non-toxic signs observed to be used for child artwork. Ensure all art materials are safe to use for children (AP/nontoxic). Guidelines for art supplies will be sent with this report. This was corrected onsite.
