      • A bin of crayons consisting of broken small pieces were observed to be stored on the bottom shelf and accessible to toddlers potentially becoming a choking hazard.The operator was instructed to rearrange the shelving, moving the bins with smaller items to higher, hard to reach places.

      • First aid kits throughout the facility were missing CPR masks.Please ensure all first aid kids are fully equipped with CPR masks.

      • 1) Peeling duct tape was used to hold down rugs located in the daycare room where younger children sit and crawl on the ground. The duct tape was observed to be peeling and trapping contaminants making it difficult to keep clean.Please remove the duct tape. If the rug curls up and becomes a tripping hazard, please remove or replace the rug entirely.2) The round rug located in the daycare room was observed to be soiled in stains.Please deep clean the rug.
