    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Hand sink in the back area did not have soap available for hand washing. Please ensure all hand sink are properly equipped with soap to ensure effective hand hygiene.
    • 18. Is reusable equipment cleaned properly (Non-Critical)?
      • Electric clippers used for haircuts had hair on the blades.Please clean clippers after use and before disinfecting so that all hair is removed from the clippers
    • 20. Is reusable equipment disinfected properly?
      • 1.The Barbicide containers used to hold combs and scissors was not filled to top, resulting in the top of equipment not being submerged. Please ensure that the Barbicide containers are filled up enough to fully submerge all equipment.2. Operator indicated that Barbicide was being replaced once a week. The manufacturer's instructions states that Baricide needs to be replaced daily, or whenever it is visibly dirty.Please replace the barbicie in the disinfectant containers are replaced on a daily basisi.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. Hair was found on the Q-tips used for waxing within the nose. Please discard of the Q-tips contaminated with hair, and store Q-tips in a sealed container to prevent further exposure to hair.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The drawer underneath the clippers on both sides of the facility had noticeable hair build up on the inside.Please clean the inside of the drawers and remove all the hair to prevent the risk of contamination of equipment.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Updated 12/15/2025Violation is still outstandingHair residues were observed in the wax container, around wax jar surfaces and on other facility's surfaces.-Please discard the wax container and do not double dip with a used wax applicator.-Also, clean and disinfect the facility's surface and ensure surfaces are in sanitary condition at all times.

      • REPEAT VIOLATION:1/31/2024:Hair residues were observed in the wax container, around wax jar surfaces and on other facility's surfaces.-Please discard the wax container and do not double dip with a used wax applicator.-Also, clean and disinfect the facility's surface and ensure surfaces are in sanitary condition at all times.
