    • 06. Are proper protocols being followed in regards to client care?
      • It was noted that the facility was performing diapering on the floor in both toddler rooms. Please acquire diapering stations for toddler rooms.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Water damage was noted on the hand washing sink cabinet located in the toddler number 1. The water damage was noted to be painted over. Please remove and replace all water damaged materials. 2. A duct tape was noted on the curtain rod in the toddler room 1. Please repair or replace the curtain rod.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1. The hand washing sink in both toddler rooms was being used for diapering and all other purposes. Please be advised that the diapering sink must only be used for diapering purposes.2. It was noted that staff members were using designated diapering sink for hand washing prior to handling food in the baby room 1
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Staff members were not washing hands between tasks in baby room 1. Importance of hand washing was discussed with the operator during the inspection. Hands must be washed between tasks.

      • A damaged diapering pad was noted in the toddler room. This diapering pad was discarded during the inspection. The operator had extra stock and placed a new diapering pad in the toddler room.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. One of the electrical switches was noted to be missing electrical switch plate. Please install the missing electrical switch plate.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Paint was noted to be peeling throughout the facility. Please paint all surfaces and ensure they are maintained in a smooth, easy to clean and moisture resistant state. 2. Edging was missing from mechanical cabinet located in toddler room. Please install the missing edging. 3. A crack was noted on the floor of one of the toddler rooms. Please repair the floor.

      • 1. One of the electrical switches was noted to be missing electrical switch plate. Please install the missing electrical switch plate. 2. The mechanical/electrical panel was not locked at the time of inspection. Please ensure that the electrical panel is kept locked at all times.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. The diapering disinfectant was noted to be at 100 ppm. Please ensure that the chlorine solution is maintained at 1000 ppm for diapering area. 2. Diapering pads were noted to be damaged in both toddler rooms. Please repair or replace diaper pads as required. 3. Diaper pads were noted to be stored on the floor and were noted to be accessible by clients. Please ensure that diaper pads are stored away from clients reach.

      • 1. Paint was noted to be peeling throughout the facility. Please paint all surfaces and ensure they are maintained in a smooth, easy to clean and moisture resistant state. 2. Edging was missing from mechanical cabinet located in toddler room. Please install the missing edging.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1. Soap dispensers were noted to be empty at the hand washing sink located in toddler 2 and washroom. Please ensure that all hand washing sinks are equipped with appropriate hand washing supplies. 2. Both soap dispenser located in toddler rooms were not in operational state at the time of inspection. Please be advised that all soap dispensers must remain operational at all times. Please repair or replace soap dispensers as required.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • General cleaning and organizing is required throughout the facility. Please clean and organize the facility thoroughly and ensure it is maintained in a clean and sanitary state at all times.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Holes were noted on sleeping supply storage bags. Please replace these storage bags.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • 1. A few paper towel holders were noted to be missing. Please install the missing paper towel holders.2. General cleaning is required in the washrooms. Please clean all washrooms and ensure they are maintained in a clean and sanitary state.
