      • Please obtain testing equipment for the high-temp dishwasher to check temperature levels.PHI had to run the dishwasher 3 times to achieve proper sanitizing temperatures.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • Food debris observed behind the cook line, please clean the area.

      • QUAT sanitizer solution prepared in washcloth buckets was weak and tested at <100 ppm. Operator refilled washcloth buckets with QUAT sanitizer. Concentration tested at 200 ppm.- As discussed, ensure that sanitizer is prepared and tested regularly using available test strips.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • Hand sink at the entrance to back kitchen was blocked off by a rice cooker and was inaccessible. Operator moved rice cooker away from the hand sink.- Ensure hand sink is unobstructed and easily accessible for staff use.
