- Observation (CORRECTED DURING INSPECTION): Rodent droppings were observed on the floor in the dry storage room and an open cabinet surface.
- Corrective Action(s): Operator cleaned and sanitized the above-noted affected areas to remove the droppings. Monitor for signs of pest activity and continue with pest control measures and regular cleaning and sanitizing of the facility to maintain it in a clean condition.

- Observation (CORRECTED DURING INSPECTION): One bag of flour observed stored in the dry storage room had holes in it. Rodent droppings were evident within this dry storage area.
- Corrective Action(s): Staff member discarded the above-noted bag of flour during the inspection. Staff member re-located the second bag of flour (observed to be in an intact condition) on a higher shelving unit of the dry storage room. It is recommended to obtain additional pest proof containers with tight fitting lids to store dry goods in sealed pest proof containers to protect them from potential contamination.

- Observation (CORRECTED DURING INSPECTION): 0 ppm chlorine sanitizer was measured in the sanitizer spray bottle. Only scented bleach was available.
- Corrective Action(s): Staff member obtained a stock bottle of unscented household bleach during the inspection and setup 100 ppm chlorine sanitizer in a sanitizer spray bottle and second compartment sink. Ensure food contact surfaces/equipment are washed, rinsed, sanitized, and air dried to maintain them in a sanitary condition.

- Observation (CORRECTED DURING INSPECTION): Rodent droppings were observed over the following items: one stack of empty pizza boxes, a box of empty take-out containers, a box filled with bags, and two lids and two containers.
- Corrective Action(s): Staff member discarded the contaminated pizza boxes, takeout containers, and box of bags during the inspection. Staff member placed the lids and containers into the ware-washing sink for manual washing, rinsing, sanitizing, and air drying.

Conduct a deep clean of the facility and sanitize the affected areas to remove the rodent droppings; Correct today. Monitor for signs of pest activity and clean and sanitize affected areas immediately. Implement a pest control program and provide a copy of your pest control report to the district Environmental Health Officer; Correct by February 16, 2026. Ensure all food contact surfaces, food equipment, and food is stored in a manner to protect it from potential contamination; Correct today. Seal all potential entry points; Correct within 1 week.
