- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
- Services were verified at the time of inspection. Re-iterated the requirement to notify this office of any new services being added and/or new beauty instruments being used.
- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- Written procedures for waxing/sugaring services and electrolysis services were completed at the time of inspection.
- 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
- Expired sterile equipment was discarded at the time of inspection. Sterility assurance documentation must be available for all equipment purchased as sterile. Expired sterile equipment is not permitted to be used.
- 26. Are cosmetic products appropriate for their intended use and being handled properly?
- Roll-on wax units are not permitted to be reused between clients. Operator agreed to remove items from facility.Operator was advised not to leave wood stick applicators allegedly being used as "stir sticks" in the waxing/sugaring pots.Cosmetic products were discarded upon request because of missing labelling requirements. Re-iterated labelling requirement for cosmetic products. The following labels are required: product identity, manufacturer, directions for use and list of ingredients.Expired cosmetic products were discarded upon request.Decanted cosmetic products were labelled with its contents at the time of inspection. The original/bulk supply must be kept on-site and labelled with product identity, manufacturer, directions for use and list of ingredients.
