- Sterility assurance documentation was not being maintained for single use items such as needles, syringes. Discussed onsite.Ensure sterility assurance documentation for single use items are recorded with client record.

- High Level disinfectant was not available in the facility to disinfect the semi-critical equipment. Obtain HLD and test strips for daily check of its efficacy. In the event there are no test strips, the HLD must be discarded everyday.
