    • 06. Are proper protocols being followed in regards to client care?
      • Toddler Room: Improper diapering procedures were observed. Staff changed children’s diapers without washing their hands between diaper changes. Children’s hands were washed by a different staff member following the diapering process. Contaminated gloves were used to handle the children’s diaper baskets. Rash cream was removed directly from the container without using a single-use applicator (e.g., Q-tip). Additionally, staff did not allow the diapering surface to remain visibly wet with disinfectant for the required one-minute contact time before wiping.Please ensure all staff are trained on proper diapering procedures and consistently follow them correctly.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Toddler room: Staff changed children’s diapers without washing their hands between diaper changes. Please wash hands after every diaper change and as necessary.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • There is a bulletin board that contains push pins/thumb tacks in the big leaders room. Please remove the push pins/thumb tacks and find an alternative method to hang paperwork and art work as push pins and thumb tacks are sharp and can be a choking hazard.
    • 08. Is this facility properly constructed (Non-Critical)?
      • There are a few corners on the white toy shelves that are damaged or missing the protection corner pieces.Please either replace the protection corner pieces or refinish and repaint the toy shelves so that they are smooth, easy to clean, and in good repair.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • There is a damaged round wicker basket holding toys in the big leaders room.Please ensure all toy containers are smooth, easy to clean, and in good repair. The director removed the wicker basket immediately.

    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • - The paper towel roll was observed on the counter, rather than in the dispenser, and was being taken out each time it was needed.- Staff will refill the paper towel dispenser.** Ensure the paper towel is always in the dispenser to avoid cross-contamination and prevent it from becoming wet when tearing off a sheet.
    • 14. Is this facility properly heated to ensure client comfort (Non-Critical)?
      • - Room #3 / Multipurpose Room: The room temperature was set to 20°C but measured as 18.6°C. The room has large glass windows.*Staff increased the temperature, and it began to rise.**The room was empty at the time of inspection. Staff should ensure the center of the room temperature is measuring to a minimum of 20°C before bringing children into the room.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - Room #4: Some dust and hair were observed in the toy bin.*Staff cleaned the area. Ensure all toys and bins are kept clean and sanitary.- Room #2: A dirty stool with hair on the footrest was stored over the children chairs.*Staff removed the stool from the area and cleaned and sanitized the children chairs.
