      • Observation (CORRECTED DURING INSPECTION): Spray bottle labelled OstroSan (Quat) measured 0 ppm.
      • Corrective Action(s): Discard and refill with fresh Quat sanitizer from the dispenser. Use test strips to verify concentration (200 ppm).

      • Observation: Sign of mice activity was observed at limited areas. Pest control company has been hired and pest management techniques have been implemented. Records reviewed. Kitchen staff were well informed and are trying to manage the issue. Live traps, sticky traps and sticky boards have been placed and indication markers noted. Floor and wall holes observed patched and use of steel wool noted.
      • Corrective Action(s):
      • - Continue monitoring mice activity and utilize pest control techniques directed by your pest management company.
      • - Clean the remaining of mice droppings on floors and sanitize areas affected as directed. Use proper PPE before cleaning and sanitizing those areas.
      • - Ensure garbage is managed properly.
      • - Ensure to block any possible entry points.

      • Observation: Mice dropping observed through out the kitchen, dry storage room, and hallway across from the kitchen. Some droppings noted at one corner of the dining room close to the kitchen. Also limited number of droppings noted on the walk in cooler floor. Cleaning and sanitatizing procedures of affected areas were discussed with head chef. Some sticky traps and snap traps was noted in the kitchen, dry storage and staff room.
      • Corrective Action(s): Following actions are to be complete immediately:
      • - Clean and sanitize all areas with obvious sign of mice activities. Pay special attention to the corners and sanitize as discussed.
      • - Floor must be clean and free of any food before closing the kitchen for the night.
      • - If noticed dropping on tables or near clean dishes, cutlery, utensils etc., rewash and sanitize implements and suspected food related equipment.
      • - Ensure there is no food left open during the night and store food in pest proof containers.
      • - Monitor mice activities and take actions as activities increases.
      • - Make aware your staff regarding the problem and utilize sanitation plan to manage the issue.
      • - Ensure walk -in cooler door sweep is fit properly and gaps at both sides are blocked.
      • Important note: Pest management plan is to be implemented by a professional pest control company. Aquire long term solutions and update your pest control plan accordingly. Keep records of actions taken.

      • Observation: Possible mice entry noted on baseboard in the dining area, under door to emergency exits in hallway and the side of walk in freezer frame as well as plumbing access holes behind the ice maker.
      • Corrective Action(s): Block possible mice entrances. Ensure to pach all holes and block possible entry points. Eliminate any gaps and hiding places in doors and ensure door sweeps are flush to the floor as much as possible.
      • Utilizing steel wool recommended.

      • Observation: Leak observed under dish washer sink/screen from the stainless steel drain board. This leak create pooling water on the floor. Also it provides water source to mice.
      • Corrective Action(s): Repair the sink so that there is no pooling water underneath the dish washer. Remove Leak catch container & keep the area dry.

      • Observation: Ice Scoop observed to be cracked and chipped. This is a physical health hazard as chipped plastic could potentially end up on ice or in the ice bucket.

      • Observation (CORRECTED DURING INSPECTION): Dishwasher contained 0ppm chlorine residual at plate surface. Discovered that bucket of chlorine sanitizer that feeds dishwasher was empty. Staff records are not being filled out correctly.
      • Corrective Action(s): Ensure that dishwasher temperature and sanitizer is monitored and recorded daily. Use test strips found onsite. Dishwasher was primed with a new bucket of sanitizer. Chlorine residual measured 100ppm at plate surface. Date to be corrected: Immediately
      • *Note: All dishware to be rewashed and sanitized.

      • Observation (CORRECTED DURING INSPECTION): A puddle of water was observed on floor adjacent to walk-in freezer. It is believed it is leaking again.
      • Sandwich preparation fridge is 6C. It was checked approx. 30 mins later with no change.
      • Corrective Action(s): Address leaking walk-in freezer.
      • Do not use sandwich preparation fridge unless it can maintain foods at 4C or colder. Potentially hazardous foods will not be stored in unit under addressed. Date to be corrected: Immediately.

      • Observation (CORRECTED DURING INSPECTION): Unable to locate an accurate thermometer in walk-in cooler.

      • Observation: Prep cooler is 2 degrees C below and maintaining foods in inserts on top at 5 degrees C.
      • Corrective Action(s): Ensure that foods in line cooler are maintained at 4 degrees Celsius at all times. Monitor carefully and adjust or repair as needed.

      • Observation (CORRECTED DURING INSPECTION): Spray bottles of sanitizer were measured and found to be less than 150PPM quat residual.
      • Corrective Action(s): It was manually mixed and still not adequate until it was mixed with a new bottle of concentrated Quat. This is a repeat issue. Once manually mixed following manufacturers instructions with a new bottle, 200 PPM Quat was achieved. Operator to have dispenser checked by chemical company. All dispensing of sanitizer MUST be checked with test strips.
