      • No hand soap was available at handwashing sink.-Ensure liquid hand soap is always available for handwashing-Main operator sent photo of newly obtained hand soap after inspection

      • Infraction for Alona Mostova Nail Master - porous wooden cuticle pushers were being disinfected in CS20.-Operator must ensure that single use items are discarded after use
    • 20. Is reusable equipment disinfected properly?
      • Disinfection tray labeled Anastasiia Lisohor had a liquid inside that was clear. It did not make CS20 test strip change colour. No reuse chart available, but there was a CS20 bottle with this name on it.-Ensure this operator has a reuse chart and is only using CS20 in the tray.

      • Velours - operator was not planning to use skin antiseptic before offering invasive service-Operator must cleanse skin and then use a skin antiseptic before offering invasive services-Operator elected to use chlorhexidine product

    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Equipment for invasive services was observed onsite. The operator had not notified the RHA regarding this service.-Ensure all services being offered are listed in the most recently submitted Personal Services Notification
    • 24. Are waste sharps disposed of properly?
      • Needles and syringes that appeared to be used (had yellow residue in them) were observed in the garbage can and in a storage cabinet.-Operator is not to use needles without a sharps container available
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1) A yellow liquid called "Shining Peel" was present in an injection vial. Manufacturer's instructions were not available for the product, and other writing on the vial was not in English.2) A large yellow tub of "J-Cain" cream was observed on the facial cart. The product had no other English labeling. It appeared to be a lidocaine-like product at 10.56%.-All cosmetic products used must have manufacturer's available-Products with controlled substances must have a Drug Identification Number-Cease use of these products until manufacturer's instructions have been provided to inspector for review
