- 1. The steps for reprocessing semi-critical instruments are not correctly followed. The operator indicated that tools are placed directly into the high-level disinfectant without prior cleaning.- Wash tools with soap and water, rinse, thoroughly dry prior to fully submerging in undiluted high-level disinfectants for the required contact time provided by the manufacturer. Rinse with water afterwards and store in a clean/dry location.Ensure that all semi-critical instruments are reprocessed following proper procedures to achieve adequate disinfection and prevent potential risk of infection transmission.2. A bottle of PREempt CS20 was expired. No unexpired bottle available to replace at the time of inspection.- Please discard and obtain an unexpired replacement.Ensure that only unexpired products are in use according to manufacturer's instructions, to ensure effective disinfection.
