- Staff were not aware of where facility-specific written procedures were located.Staff were not aware that the high-level disinfectant was expired.

- The alcohol-based hand rub (ABHR) was expired. PSW claimed the ABHR was being refilled but did not have original container to verify label requirements.Single-use towels were not available at the bathroom handsink.

- Used single-use items such as nail files, nail buffers, waxing strips and wooden wax applicators were not being discarded after each use.

- Clean tools were being stored in dirty storage areas. Refectocil tints were being stored in a glass jar that was visibly soiled with tint residue.Used glass dapper dish had build-up of refectocil tint used for eyebrow tinting services.

- The linens on the waxing bed was visibly soiled with a dirty footprint.

- High-level disinfectant was expired.There were no high-level indicator tests strips on-site.Facility is not maintaining high-level disinfectant logs.There were two spray bottles that had been refilled with low-level disinfectant. The spray bottles were not properly labelled with its contents.The disinfectant tray was too small to fully immerse the cheese-grater style metal foot files. Obtain a larger disinfectant tray.

- Staff claimed they were discarding the used credo blades into the regular garbage. There was a sharps container under the reprocessing sink located in the corner. The sharps container was re-located so it is easier accessible for staff.

- Decanted cosmetic products in various areas throughout the facility need to be properly labelled with its contents. The original/bulk supply must be kept on-site and labelled with product identity, manufacturer, directions for use and list of ingredients.Various cosmetic products used for eyelash extension services were missing labelling requirements. It did not have instructions for use or list of ingredients.Cosmetic products are not being stored properly to keep them sanitary and safe for used. There were open packages of eyelash extensions.Refectocil tints were missing caps which could contaminate the bulk supply.Bulk supply of vaseline was visibly contaminated with tint used for eyebrow tinting service.

- The facility was not maintained in a visibly clean state. Manicure stations had a build-up of nail dust. Storage areas used to store clean equipment was visibly dirty.
