      • The operator was observed using a low-level disinfectant to clean pedicure basins. The operator was instructed to use a high-level disinfectant.

      • Several nail polishes were identified without ingredient lists or manufacturer instructions. All cosmetic products must be clearly labeled with a complete list of ingredients and appropriate usage guidelines.

      • Cosmetic products intended for use on eyelashes were found without manufacturer instructions or ingredients in reprocessing room. The operator informed the PHI that the items were not in use. Thus, products were immediately discarded by operator.

      • Written procedures were not completed during inspection for accidental skin cut or puncture, cleansing of skin, and decontamination of dual purpose sink in waxing room. - The above written procedures are required to be completed and easily accessible onsite.

      • Written procedures were not available onsite during inspection for accidental skin cut or puncture, cleansing of skin, and decontamination of dual purpose sink in waxing room. - The above mentioned written procedures are required to be completed and easily accessible onsite. Templates provided to operator via email on August 16th, 2024.

      • Visibly soiled plastic single-use mascara applicators were noted in a drawer in the waxing room. - Do not reuse single-use items that cannot be properly cleaned and disinfected. Used applicators were discarded by operator.

      • Visible debris noted on scissors and tweezers stored in a drawer in the waxing room. - Ensure reusable equipment is stored in a clean manner that protects equipment from contamination. Operator moved scissors and tweezers to reprocessing area to clean and disinfect equipment.

      • High level disinfection testing strips expired in April 2024. - Obtain valid test strips for measuring high level disinfection concentration.

      • Operator reported that metal drill bits at manicure stations were cleaned with alcohol only. Nail nippers and clippers with visible debris noted at various stations. - All semi-critical equipment must be cleaned and disinfected with a high level disinfectant as per manufacturer's instructions. Drill bits, nippers, and clippers were removed from the stations by operator and moved to sink for reprocessing.

      • As per operator, liners are discarded, then pedicure basins are cleaned with Scrubbing Bubbles cleaner prior to next client. - Pedicure basins should be cleaned and disinfected after each use with a low level disinfectant at minimum even when single use disposable liners are used. Impeller covers should be removed for proper in place cleaning and disinfection. An intermediate level disinfectant is recommended and may be more effective in preventing biofilm accumulation. Ensure to clean and disinfect pedicure basins immediately after use. Used plastic liners were discarded and pedicure basin cleaned and disinfected by operator.

      • No high level disinfection log was available onsite during inspection. - High level disinfection logs are to be maintained daily during operation to ensure adequate disinfection of semi-critical equipment. High level disinfection log template provided to operator via email on August 16th, 2024.

      • Cosmetic items with no labelling, inadequate labelling such as missing ingredients, instructions for use, and/or manufacturer information, or labelling in other languages were noted at various stations.- Ensure all cosmetic products used within the facility are properly labelled with instructions for use and ingredients. Operator removed noted items to be discarded or for personal use.

      • Unlabelled spray bottles noted at various stations throughout the facility reported to contain alcohol as per operator. - Label all spray bottles for proper identification of contents and prevention chemical hazard exposure.
