- bleach spray bottles were not labelled in the baby room and in the toddler room. Director corrected this

- Baby room: Staff indicted that they plate and warm parent provided food then wash the dishes with just soap and water in the handsink. The handsink is the one adjacent to the diapering stand. Re useable dishes and children water bottles must only be processed via wash, rinse, sanitize steps in the kitchen three sinks. Discussed with the operator.

- 04. Are clients at this facility being protected from hazards (Non-Critical)?
- Ice melt crystals bucket stored in one of the room next to the exit door. This was corrected during the visit
- 06. Are proper protocols being followed in regards to client care?
- Toddler room safety belt for diapering was observed to be in disrepair. The belt was repaired with electrical tape. Ensure safety belt is in good repair, easily cleanable, impervious to moisture and safe for client use.

- Mattresses used for sleeping were observed to be in disrepair. Some of them were taped with electrical tape and some had corners that were not impervious to moisture and easily cleanable.Mattresses were observed to be covered with covers that were not impervious to moisture (i.e. cloth).
