      • Paper towel was not available in dispensers throughout the facility.Facility was waiting on an order of paper towel that would fit in the dispensers.An alternate source of paper towel (Eg regular paper towel in countertop dispensers) may be needed until the order arrives. Paper towel must be available in dispensers at each handwashing sink at all times.

      • A chair near the diapering station in the toddler area is cracked in several places. The chair was off to the side.Remove the chair from the facility as it poses a pinch/cut hazard.

      • The transition between the foyer and the OSC area is missing some flooring, creating a gap and tripping hazard. The surface is also not smooth, impervious to moisture, and easy to clean.Repair the transition area to ensure the floor is smooth, impervious to moisture, and easy to clean.

      • One spray bottle labeled "bleach" measured 0 ppm chlorine.A 200 ppm solution was prepared during inspection.Ensure chlorine bleach solutions are maintained at 100-200 ppm. Verify concentration with a test strip.If solutions are used over multiple days, verify concentration with a test strip each day prior to use.

      • The roof is leaking in several places, including in the foyer and in the main child care room. Mitigate the cause of the water infiltration.Water damaged materials must be removed and replaced with non-damaged materials.

      • The transition between the foyer and the OSC area is missing some flooring, creating a gap and tripping hazard. The surface is also not smooth, impervious to moisture, and easy to clean.Repair the transition area to ensure the floor is smooth, impervious to moisture, and easy to clean.

      • Play doh was found in a cupboard at room temperature. Inspector was informed it was store-purchased play doh.Once used, keep play doh in the fridge to help prevent bacterial growth.If using homemade play doh, flour cannot be used as an ingredient. Keep a recipe available to indicate any homemade playdoh ingredients.

      • 1. Vinyl flooring in the main part of the child care space has shifted and there are significant gaps present.Ensure the flooring is repaired or replaced and is smooth, impervious to moisture, and easy to clean.2. Carpet on the stage area has torn.Replace or repair the carpet so it is in good repair and cleanable.

      • The basement had dampness along the walls and along a crack in the floor near the stairs. The crawl spaces consist of clay and are wet.Upstairs did not smell musty. A dehumidifier was present but not connected or operational.Basement must be thoroughly dried. If water continues to infiltrate further action may be required.Continue to monitor.

      • 1. Vinyl flooring in the main part of the child care space has shifted and there are significant gaps present.Ensure the flooring is repaired or replaced and is smooth, impervious to moisture, and easy to clean.2. Facility has had difficulty sourcing a new tile.Ensure the tile is replaced or the opening otherwise covered.

      • There was no paper towel available at the handwashing sink in the baby area.Ensure paper towel is always available for drying hands. Keep extra paper towel readily available or replace immediately if it empties.Paper towel was replaced at the handwashing sink during inspection.

      • The basement had dampness along the walls. There was standing water present near the sump pump, which was recently replaced. A small fan was blowing air into the crawl space with the sump pump.Basement was empty of stored items.Basement must be thoroughly dried. If water continues to infiltrate further action may be required.Physically removing the standing water (eg with a shop vac) and installing a dehumidifier are recommended.

      • Some of the water bottle labels in the child care area had worn off and were difficult or impossible to read.Ensure all water bottles are labeled with the child's name and that the name is easy to read.

      • Bingo daubers and shaving cream specifying "keep out of reach of children" were found in a cabinet within reach of children.Use only non toxic arts and crafts materials, preferably labeled as "non toxic" or with the "AP" approved product seal.AHS guideline document Arts and Crafts Safety provided via email.Homemade play doh was observed at room temperature.Store play doh in the fridge between uses to help prevent bacterial growth.

      • A report was received that the roof of the facility is sound and solid; however, the caulking has failed in a few places which is causing the leak.The contractor recommended resealing the flat section of the roof with Butyl seal.The work cannot be completed until the weather warms up.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • The basement had several significantly sized wet/damp spots. There was a small amount of standing water present near the sump pump.Basement was empty of stored items.The sump pump has been replaced.Basement must be thoroughly dried. If water continues to infiltrate further action may be required.Report received that all three furnaces have been serviced and are in good working order. Cold air returns are present on the main floor with no intakes in the basement or crawl space.

      • Roof tiles were removed from a section of ceiling in the central boot storage area. No water was dripping at the time of inspection.It was not raining at the time of inspection.It was reported the roof was inspected and the leak is suspected to be caused by dry and cracked silicone. Silicone will be replaced.Provide a copy of the invoice and/or report to show the work has been completed.

      • Roof tiles were removed from a section of ceiling in the central boot storage area. A bin with a few inches of water in it was present beneath the open tiles.No water was dripping into the bin during inspection and surfaces did not have elevated moisture readings. It was not raining at the time of inspection.Have the roof inspected to ensure it is in good repair and does not leak.

      • The basement, which is used for storage and also contains the furnace, was flooded. Approximately 2-3 inches of water appeared to be present. The sump pump possibly failed.Remove all water. Any water damaged materials must be appropriately cleaned and sanitized, or discarded.Have the furnace inspected once the basement is dry to ensure it is functioning properly.Repair the sump as necessary.

      • 1. Cups were being washed but not sanitized. Cups are for individual use.Ensure water bottles and cups are washed and sanitized at least once per day.After washing and rinsing, submerge cups in 100 ppm chlorine bleach for 2 minutes and allow to air dry.2. Several water bottles were not labeled. Ensure all water bottles or cups are labeled with child's name. If child is using an interim bottle, use tape or another temporary method of labeling.

    • 06. Are proper protocols being followed in regards to client care?
      • The diapering station in the middle section did not have child restraints present. The sides measured 2" from the pad. The diapering pad was too large for the diapering station with overhang on the front, back and sides.Replace the diapering pad with one that is sized appropriately. There should be no or very minimal overhang.The sides may not be sufficient to prevent children from rolling off. Ensure an adequate child restraint is present and used.

      • Bleach bottle near the diapering station in the infant area measured 0 ppm.Bottle was replaced during inspection.Ensure bleach bottles are maintained at a minimum of 100-200 ppm. Verify concentration with a test strip each time a solution is prepared. If solutions are used over multiple days, verify solution concentration each day the solution is in use.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • 1. The drawer and basket at the diapering station were cloth and not easily cleanable or able to be effectively disinfected.Remove the cloth drawer and basket and/or replace with items that are smooth, impervious to moisture, and easy to clean.Ensure all elements of the diapering station are able to be effectively cleaned and disinfected.2. Two chemical bottles had worn or missing labels to identify contents.Ensure all chemical bottles are clearly labeled to specify contents.
