- Operator was maintaining the client records, however sterility documentation corresponding to client was not included in the client information.Discussed with the operator to keep the label from sterile needles indicating the lot number along with client records.

- It was observed that multiple disinfectant used in the facility were expired.Please make sure to follow the manufacturer's instructions for use of disinfectant and expiry dates.
