    • 05. Do food handlers refrain from handling food while ill and have acceptable attire and hygiene?
      • No - the operator did not wash hands after returning from outside of the food premises unit. Staff (food-handlers) must wash hands at the designated hand-wash sink after returning from the washroom, garbage disposal, and after all other activities outside of the food premises (i.e. prior to beginning food preparation and/or cooking).
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • No - there were about ten (10) captured German cockroaches on the glue-board traps in the food premises unit, at the rear food prep kitchen area (near the entry door) and at the front food service area (at far end corner). The operator is to ensure continued pest control service and daily monitoring (along with floor cleaning), with the 'affected' glue-boards removed/discarded (until there is no further evidence of this insect activity).
    • 23. Is the facility maintained in a clean and sanitary condition?
      • No - better organization of stored items and equipment in the food premises unit is necessary to allow for ease of inspection, pest control monitoring, and cleaning. Also, there was accumulation of a 'white salt-like substance' on the flooring at the rear entry doorway area, front storage area, and in behind the unused ice machine and dry food storage areas. The operator is to ensure adequate floor washing is regularly completed daily (and weekly in behind larger equipment) to remove such accumulation of material and any dirt/dust.

      • No (repeat violation) - there was insufficient cleaning and sanitation under the main cooking area, with significant grease/oil accumulation and some observed insect activity on glue-boards.The operator was directed to work with the shopping centre operations managers and licensed pest control company to mitigate and prevent insect activity in the food premises unit (which includes maintained cleaning and sanitation throughout the food premises).

      • No (repeat violation) - some cleaning had been completed along the main kitchen wall behind the cooking equipment, though there was still a lack of thorough cleaning necessary at the main grill and cooking line, on the floor under and around the food equipment (as there was significant accumulation of grease/oil deposits in this area).The operator is to conduct the required cleaning, or contact the franchise headquarters, and/or contact the services of a cleaning company again to ensure a thorough 'deep cleaning' has been completed. Better overall organization of stored food products, equipment, and other items is necessary to ensure ease of cleaning and inspection in the food premises, along with a written cleaning/sanitation plan (AHS template provided to the operator via email).Also, a food premises unit maintained in a clean and sanitary condition will allow for better pest control service and treatment (and ensure insect activity is mitigated and prevented, including any impacts on the adjacent food premises in the food court).

      • No - there was noted improvement with some German cockroaches (less than ten) and some drain flies observed on two glue-boards on the floor areas at the main kitchen line. There has been overall improved cleaning and sanitation of the food premises unit.The operator is to continue to work with Building Management and licensed pest control company to mitigate insect activity in the food premises unit. Also, the operator was advised to add some chlorine bleach into the floor and sink drains at end of evening work shift (to prevent insect growth overnight inside drains).

      • No - there were about ten (10) German cockroaches observed on two glue-boards in the rear kitchen area, along baseboards under sinks and storage shelving.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • No - there was some accumulation of food debris, dust/dirt, and other material (including two glue-boards with captured cockroaches) on the floors under sinks and storage shelving in the rear kitchen area. These hard-to-reach areas are to be more regularly and thoroughly washed (at least once per week), which will increase overall sanitation and prevent any continued insect activity.
