- Observation:
- Yearly health decal is expired (Mar-31-25) as yearly health permit fees, of $150, have NOT been paid. Hand delivered outstanding permit fees letter.
- Corrective Action(s):
- Immediately pay the yearly health permit fees of $150. Failure to do so will result in the closure of the premises until they are paid in full.

- 1) Wash and clean the floor around and beneath the deep fryers to remove the grease. Staff shall do this every night before they leave.

- Observation:
- Mice droppings found on the floor in the dry storage food area and under the fridge in the sushi bar. Mice droppings carry diseases.
- Corrective Action(s):
- - Clean up the mice droppings and clean all surfaces with hot soapy water, and then spray with a bleach sanitizer solution.
- - Hire a professional pest company to eliminate the mice and to block all openings or holes to prevent entry of the pests.
- - Increase the frequency detailed cleaning is done, and of hard to reach areas.

- Observation:
- 1) Accumulated grease and food on the walls and floors around, beneath and behind the deep fryers and cook stations. This provides food for mice and will attract them to the premises.
- 2) Food debris and stains found on legs of equipment, tables and sinks; and spilt food on shelves and on the floor beneath & behind equipment. This will attract flies and mice.
- 3) Too many items are being stored on the floor and in the premises, this makes it difficult to clean the floor correctly and it provides places for mice to hide.
- Corrective Action(s):
- 1) Wash and scrub these areas EVERY night to remove all the grease and food. Use hot soapy water and degreaser to remove the food and grease. Move equipment and racks/shelves away from walls so the floor and wall can be scrubbed cleaned. Then sanitize with a bleach solution to kill germs and remove body oils left by mice.

- Observation:
- Found some mice droppings and shredded paper in dry storage area and on various shelving units and racks. Mice and their droppings carry diseases.
- Corrective Action(s):
- 1) Immediately clean up the droppings. Wash and sanitize all areas that have come into contact with the droppings, with hot soapy water and then spray with bleach sanitizing solution.
- 2) Store all foods (packages of food, onions, squash, cabbage, etc.) in hard plastic, glass, or stainless steel containers with tight fitting lids.
- 3) Throw away cardboard, magazines, newspapers, and any items that you do not need to store or use in the premises. The mice will shred these items and use it to make a nest.
- 4) Remove and/or throw away extra items that do not need to be stored in the premises. Decluttered the premises to eliminate places for mice to live and hide, and to decrease the materials mice will use to make a nest.
- 5) Detailed cleaning and cleaning of hard to reach areas require to be done more frequently. The legs of equipment, storage shelves, tables and sinks; floors and walls behind and under equipment, sinks, and shelving racks; and shelves and racks all require to be scrubbed clean with hot soapy water to remove food and grease. This shall be done at least weekly, if not daily.

- 2) Accumulation of grease found on the exterior surfaces of cooking equipment (flat grill, deep fryers, etc); on the wall behind the cooking equipment; and on the floor beneath the cooking equipment. This will attract pests.
- Corrective Action(s): 1) Remove all items off the shelves and floor int he sushi making area, and wash all these surfaces with hot water and soap to remove food debris. Rinse with hot water and then sanitize. Ensure this is being done weekly.
- 2) Scrap and wash exterior surfaces of cooking equipment, walls and floors beneath & behind cooking equipment to remove the grease. Use hot water, soap/degreaser and scraper/brush. Rinse with hot water and sanitize. Ensure this is being done at least twice a month, ideally every week.

The grease and food will attract pests.
- Corrective Action(s): Immediately wash and sanitize these surfaces to remove the grease and food. Using a degreaser (Vim, Power & Shine Kitchen cleaner) will make it easier and faster to remove grease on surfaces. The exhaust hood filters should be cleaned at least once a month (or more).

- Observation: 1) Found protective plastic covering the ice machine and the back kitchen assembly cooler. The plastic is not cleanable and its purpose is only to protect the machines from scratches during transit.
- 2) Found a metallic type material or wall paper covering the counter tops around the two, front hand sinks; the top and sides of the dishwasher; and on the wall above the 3-compartment sink. This material is not food grade, durable, sealed, or cleanable.
- 3) Found wall paper covering the wall adjacent to the deep fryers, and the wall paper is covered in food. The wall paper is not food grade, durable, sealed, or cleanable.
- Corrective Action(s): 1) Immediately peel off all the protective plastic covering the mentioned surfaces, and use a degreaser to clean and remove the glue.

- Corrective Action(s): Repair the walls so that they are durable, smooth, level, sealed and cleanable. Install drywall to cover all the openings and the studs. Seal the cracks between the drywall sheets with drywall mud; sand; and then paint.
