- Aluminum foil covering the deep fryer and wire racks in the kitchen, have accumulated grease and food. This will attract pests.
- Corrective Action(s):
- The aluminum foil must be removed and thrown away EVERY night, and new foil installed. If this is not followed, stop using foil to cover surfaces.

- 1) Low temperature dishwasher had a [Cl2] of 0ppm. The dishwasher is not sanitizing and killing germs on the dishes. This could cause people to become ill. During the inspection, staff called a repair company to fix the dishwasher. They will be arriving in 1 hour.
- 2) Drain pipe under the 2-compartment sink, beside the dishwasher, is leaking. Dirty water is pooling on the floor, which will attract and breed flies.
- Corrective Action(s):
- 1) Immediately call a repair company to fix the dishwasher, so the [Cl2] is maintained between 50-100ppm.
- 2) Immediately stop using the dishwasher. OR, manually sanitize dishes in a sink filled with sanitizer (mix 1Tbsp of bleach with 4L of hot water), after they have been placed through the dishwasher.
- 3) Purchase chlorine test strips from the repair company, so staff can test the concentration of the sanitizer at the beginning of each day.

- Observation (CORRECTED DURING INSPECTION):
- The assembly cooler in the front waitress area, storing side dishes and sauces, was broken. The temperature of the food inside was 28C. This temperature is too warm and will allow bacteria to grow in food, which will make people sick when they eat it.
- Corrective Action(s):
- Immediately thrown away the perishable sauces containing mayonnaise, and move all low risk foods into a working cooler. At time of the inspection, 3 bottles of sauces were discarded and the low risk food were moved into the front bar cooler that was 4C.

- Observation (CORRECTED DURING INSPECTION):
- The cooked rice in stainless steel containers were 54C in the hot holding unit. The temperature is too cold, bacteria will grow in the rice and people could get ill eating it. The rice had been in the hot holding unit for 1.5 hours.
- Corrective Action(s):
- Immediately increase the temperature to 60C and maintain this at all times. During the inspection, staff increased the temperature dial to 60, and by the end of the inspection the temperature of the rice was 60C.

- Observation (CORRECTED DURING INSPECTION):
- Mounted soap dispensers in the kitchen at all hand washing sinks are broken and do not dispense soap. One paper towel dispenser was empty as well. Staff are unable to wash and dry their hands correctly.
- Corrective Action(s):
- Immediately fix the soap dispensers or place a working soap dispenser at each hand washing sink that has a broken dispensers. At time of inspection, staff placed a working soap dispenser beside each hand washing sink.

- Observation:
- 1) Found mice droppings and scent trails in the bar area on the floor under the bar, and on the top counter top of the bar. Also found droppings under and behind the ice making machine. Droppings carry diseases and are unhealth for staff to be around and breathing in.
- 2) Fruit/drain flies found in the front bar area and the dishwashing area. Flies could spread disease on their feet.
- Corrective Action(s):
- 1) Clean up the droppings and the surfaces they have touched with hot soapy water, and then spray with bleach sanitizer (1 part bleach and 9 parts water). These areas shall be cleaned every night to monitor for new droppings/activity and to eliminate safe runways.

- 2) A thorough cleaning of the floors, undersides of sinks, and the walls under/behind the dishwasher and the bar sinks shall be done to remove any food, grease and pooling water. Cleaning of these areas shall be weekly to remove any food and water source for mice and flies eat and breed in. Have a professional pest company treat all sink drains and floor drains with a special cleaning foam to remove grease and food.

- Observation:
- There are gaps at the bottom of exterior doors that lead to the outside. These gaps are large enought to let mice in.
- Corrective Action(s):
- Immediately purchase and install door guards/door sweeps to the base of all exterior doors to prevent mice from entering.

- Observation:
- 1) Accumulation of dust on the ceiling tiles and upper walls in the kitchen. The dust could fall into food and contaminate it.
- 2) The exhaust canopy filters have accumulated grease and dust on them, especially the main kitchen canopy. The grease and dust could fall into food and attract pests.
- 3) Bar area is cluttered with items being stored on the floor and shelves/cupboards not being cleaned every week.
- Corrective Action(s):
- 1) Clean the dust off the ceiling and the upper walls. Cleaning of these areas shall be done on a regular schedule of once a month.

Throw away all un-needed or unused items and store all items 6" off the floor.

- 1) The front, waitress assembly cooler (storing side dishes) was 28C. The cooler is not maintaining 4C or colder and will allow bacteria to grow in food. Staff stated that the machine stopped working in the morning and they have called a repair person, who will be arriving very soon.
- 2) The soap pump/dispenser in the dishwasher is NOT injecting soap into the machine. The dishwasher is not washing and removing food and grease from dishware.

- 1) Immediately call a repair company to fix the cooler so it maintains 4C or colder at all times. Discard all perishable foods, and move low risk foods into a working cooler. During the inspection, the repair person arrived.

- Observation:
- Found mice droppings in the front service area under the hand washing sink; on the lower shelves below the flat grill; on the floor in the walk-in-cooler; and on shelves of cupboards. Mice and their droppings carry diseases that could be transferred onto food.
- Corrective Action(s):
- 1) Clean up all the mice droppings, and wash & sanitize all surfaces that came into contact with the droppings.
- 2) Block all openings and holes in walls and around pipes.
- 3) Hire a professional company to eliminate the mice.
- 4) Increase the frequency which detailed cleaning and hard to reach areas are cleaned.

- Observation (CORRECTED DURING INSPECTION):
- 1) Container of food stacked on top of uncovered frozen bubolgi. The bottom of the container will put germs onto the food, and this could make staff sick.
- 2) Staff making sauce on the stove was tasting the sauce by dipping his gloved finger into the sauce and then licking his finger. He then dipped his gloved finger into the sauce again and tasted it. The germs in the staff's mouth has contaminated the sauce, this could make people sick.
- Corrective Action(s):
- 1) Immediately place a lid on the container of bubolgi to protect it from contamination. Retrain staff to properly store food in containers and cover it, before stacking the containers. During the inspection, staff placed a lid on the container of bubolgi.
- 2) Immediately discard the sauce and retrain staff on the proper methods to taste food. During the inspection, the sauce was thrown away.

- Observation: Accumulated food debris and grease found on the floor under the spice rack (in the cooking area); on the exhaust filters and hood; on the deep fryers and their dividers; on the table the flat grill sits on; and on the exterior surfaces of non-perishable food containers. This will attract pests.
- Corrective Action(s): Wash and clean the mentioned areas today, and increase the frequency these areas are being cleaned. The exhaust hood, the table the flat grill sits on, and the deep fryers and their dividers shall be washed every night. The exterior surfaces of food containers shall be cleaned every week. The exhaust filters shall be washed every month.

- Observation (CORRECTED DURING INSPECTION): Observed male cook wearing gloves to complete multiple tasks without changing gloves after completing each task. The gloves are NOT washable (don't wash hands with gloves on) and they will spread germs if they are not changed after each task.
- Corrective Action(s): Immediately have the staff member remove the gloves, wash their hands and put a new pair of gloves on. Retrain staff on the correct method of using gloves. If staff fail to use the gloves correctly, the use of disposable gloves will NOT be allowed in future. DUring the inspection, the staff member changed their gloves.

- Observation (CORRECTED DURING INSPECTION): The handwashing sink in the waitress area and adjacent to the walk-in-cooler had buckets and ice scoops stored in the sink basin. Staff are not able to wash their hands as the sink is being blocked.
- Corrective Action(s): Immediately remove the items in the mentioned sink basins, and educate staff to not place or block items in the handwashing sinks. During the inspection, items were removed from the sink basin.

- Observation (CORRECTED DURING INSPECTION): 1) Found containers of food, in the walk-in-cooler (been sprouts, soup) and prep/assembly cooler in the cookline, not covered. Germs or foreign objects could fall into the food, contaminating it.
- 2) Foods and clean food equipment (colindars & bowls) are being stored on top of the grease interceptors. Grease interceptors are "dirty" as it stores oily water from the dishwasher and sinks. If the grease interceptors leak/break, the contents will contaminate the food and food equipment.
- 3) Clean cutting boards and food container & lids are being stored on the floor. These items can become contaminated by staff walking by or dirty water falling on it.
- Corrective Action(s): 1) Immediately cover all food containers with tight fitting lids, saran wrap or aluminum foil. During the inspection, staff covered all food containers.
- 2) Relocate the food and food equipment so it is stored in a clean, dry area that is 6" off the floor. Only store cleaning chemicals, pesticides, and non-food items on top of grease interceptors.

- iii) There is a divider wall, beside the ice machine in the waitress area, that is blocking access to the lower shelves. The area behind the divider can't be accessed to clean.
- iv) Floor tile in the dishwashing area is cracked and lifting up. Water is getting under the tiles.
- ACTION: 1. Install baseboards to the wall and seal them at the top + base to prevent food and water getting behind the baseboards.
- 2. Remove the divider wall and clean the area behind it. Do not put the divider wall up again.
- 3. Remove all damaged or lifting tiles from the floor and replace with new ones.
