      • Observation:
      • 1) Rodent bait/poison found in the kitchen and dry storage room. Rodents will spread the bait/poison on food and/or food contact surfaces via their hands and feet, when they touch the bait and then walk around the premises.
      • 2) Ziplock bags of food, stored in the dry storage room, can easily be gnawed through by mice. Mice will damage and/or contaminate food that will need to be thrown out.
      • Corrective Action(s):
      • 1) Immediately remove the bait/poison and stop using it inside the premises. Only mechanical means of eliminating pests (glueboards, tin box, snap traps, etc.) shall be used inside to eliminate rodents. Inform your professional pest company to NOT place bait/poison inside the premises.
      • During the inspection, the bait/poison was removed.
      • 2) Store all foods in food grade plastic, glass, or stainless steel containers with tight fitting lids to keep rodent outs.

      • Observation:
      • 1) Empty cans and debris found under the keg coolers in the bar area (by the recycling bins). This will attract and/or harbor pests (rodents, flies, etc.).
      • 2) There is accumulation of flour on doors of upright coolers & freezers, walls, and windows around the pasta making machine. This will attract pests.

      • 1) Clean the floor under the keg coolers in the bar area and ensure this is done daily to remove any debris or empty cans/bottles.

      • Observation:
      • Found openings in walls (by baseboard heater in washroom area, and by ice machine) that are possible entry points for mice.
      • Corrective Action(s):
      • Do a thorough inspection of all walls and areas around water and electrical pipes exiting walls for holes or openings that are 1/4" of larger. Block and/or tightly stuff holes and openings with steel wool; install metal plates or wire mesh (1/4" of smaller) to exclude mice.

      • Observation:
      • Accumulation of food debris and grease on legs of equipment, prep tables, sinks, and shelving racks; on walls behind equipment, prep tables, sinks, and shelving racks; on the undersides on sinks; and on the floors along the base of walls. This will attract and harbor mice and flies.
      • Corrective Action(s):
      • A deep, thorough clean of the mentioned areas requires to be done. Move equipment, prep tables, and other items off floors and away from walls to clean the mentioned areas. Use a scrub brush, hot water, soap and/or degreaser to remove food and grease debris. A thorough clean should be done on quarterly or more frequently.

      • Observation:
      • 1) Accumulation of dust on the ceiling around air diffusers and on electrical lines. The dust could fall into food and contaminate.
      • 2) Food and debris found under coolers in the bar area. This will attract pests.
      • 3) The exhaust canopy has some accumulation of food and grease. Service sticker indicates professional cleaning was due in January 2023.
      • Corrective Action(s):
      • 1) Wash and clean the dust off of the mentioned areas. Increase the frequency these areas are cleaned.

      • 2) Sweep and mop the floors, all the way to the back wall, under the coolers in the bar area. If possible, move the coolers away from walls so it is easier to clean the floor underneath them. Ensure this is being done daily.

      • Observation:
      • 1) The wood frame, around the pass-thru window, in the dishwashing area is water damaged and is no longer sealed and cleanable. Food debris will be absorbed and attract pests. This was mentioned in the previous inspection and has NOT been addressed.
      • 2) The wall, behind the mobile pasta machine, is damaged from the metal platform hitting it. The wall is no longer sealed or cleanable.
      • Corrective Action(s): 1) Replace the damaged wood frame with a material that is durable, smooth, level, sealed and cleanable.

      • Observation: Premises has an expired health permit decal, the premises is illegally operating since April 2022. The yearly health permit fees have not been paid. Hand delivered outstanding health permit fees letter.
      • Corrective Action(s): Immediately pay the outstanding permit fees. Failure to do so will result in the closure of the premises until the fees are paid in full.

      • Observation: There is a missing cold water tap handle on the hand washing sink, a clamp is being used to open and close the tap. The clamp is not food grade and not cleanable.
      • Corrective Action(s): Install a new tap handle on the hand washing sink so the cold water can be easily turned off and on, and the handle can be easily cleaned.

      • Observation: Few numbers of rodent droppings observed underneath on over, prep table and dry storage rack in dry storage area. Pest control service attended every month, but the pest control report not kept on site. Rodent infestation in food prep areas may contaminate food items, equipment, utensils, and other food contact surfaces, and cause foodborne illnesses.

Then, continue to monitor for pest activities and ensure that all food items are stored off floor.
