      • Preschool room - The gold metal pole on the wooden play equipment in the preschool room was broken and taped. Please repair or remove.> Pole removed but protrusion hazard still exists. Remove gold knobs. April 21 2026

      • A bottle of bleach solution was labelled as "sanitizer" in the preschool bathroom and also OSC room. Please ensure all sanitizers are labelled with their contents.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • There was 0ppm bleach concentration in wiping cloth solution in the OSC.There was 0ppm bleach concentration in wiping cloth solution in the baby room.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The surface of the small train table was worn in the preschool room.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • Gain soap, disinfectant, windex (glass cleaner) were stored in unlocked lower cupboards (underneath handwashing sink) in the toddler and infant rooms.Operator moved these items into a separate area inaccessible to the children in order to prevent accidental ingestion. Ensure to use and store chemicals safely either keeping them locked in lower cupboards, in a locked designated chemical room or upper cabinets.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • The miniature toy house was observed to be in disrepair with sharp edges, this issue was observed to be temporarily fixed with a masking tape.Toys with sharp edges poses a hazard to children, potentially causing accidental cuts or other types of injuries when playing with such toys. Operator discarded the miniature toy house. Ensure to maintain all toys in safe working condition to prevent hazards to children.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • The handwashing sink faucets in the preschool and OSC rooms were observed in disrepair (loose base).Staff member mentioned they are planning on replacing the faucets due to pressure applied on the handle by users. Ensure all plumbing fixtures are maintained in safe working condition and free of defects.
