    • 06. Are proper protocols being followed in regards to client care?
      • Spray bottles containing sanitizer solution are labeled and placed in each room. 100ppm chlorine solution is to be used for disinfecting diaper change tables, tables and other surfaces. 1000ppm chlorine solution is to be used for the decontamination procedure for rooms with one sink for diapering and other uses. The following rooms were found to have less than 100ppm chlorine solution in the labeled spray bottles:-Navigator 2 - bleach solution was 50ppm-Explorer 3 - bleach solution was 50ppm-Explorer 2 - bleach solution was 50ppm in white spray bottle-Pioneer 1 - bleach solution was 50 ppm- Stargazer 1, 2, 3, Stellar 1, Explorer 1, 4, and Navigator 3 were initially measured at 50 ppm, and only reached the required concentration of 100 ppm after being sprayed approximately 30 times, suggesting water or old solution was stuck in the spray hose. The following rooms were found to have less than 1000ppm chlorine solution in the labeled spray bottles: -Explorer 2 - bleach solution was 250ppm-Explorer 3 - bleach solution was 500ppm-Stargazer 1 - bleach solution was 500ppm-Stargazer 2 - bleach solution was 250ppm-Stargazer 3 - bleach solution was 500ppm-Explorer 1 - bleach solution was 250ppm-Explorer 4 - bleach solution was 500ppmThe following rooms were found to have chlorine spray bottles labelled 100 ppm that did not work: -Navigator 3 - left bathroom bleach solution labelled 100 ppm would not spray- Explorer 1 - right bathroom bleach solution labelled 100 ppm would not spray*Mix and test the bleach/chlorine solutions daily to ensure that they meet 100ppm or 1000ppm concentration as required. Please fix or dispose of the spray bottles that aren't working.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The high chairs in Stargazer 1 have sticker labels on them that are worn and peeling, and have accumulated food debris. They are no longer able to be cleaned and sanitized properly. *Please fully remove the stickers and glue so the high chairs can be cleaned appropriately.2. One of the diaper mats in Explorer 4 is becoming worn and interior foam is exposed. It can no longer be appropriately cleaned and sanitized. *Please repair or replace diaper mat.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • In Explorer 1 the left door of the toy kitchen cupboard was broken in a way that left two long screws exposed. *Please remove this toy from the room immediately and do not return it until it is fixed.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • The following rooms had toys or play equipment that was dirty or in poor condition:-Stargazer 1 and 3 - some foam toys are chewed and discoloured-Stargazer 1 - orange and yellow toy vehicle had dirt/sand in the corners-Explorer 4 - toy stove had dirt in the cracks and baby dolls are dirty-Explorer 1 - toy car raceway has dirt/sand in the corners*Please dispose of foam toys that cannot be cleaned/sanitized properly anymore due to chew marks or discoloration. Please thoroughly clean/sanitize play equipment including in cracks, corners, and hard to reach areas.*Ensure that toys are properly washed, rinsed and sanitized a minimum once per week through use of the dishwasher or the sinks in the kitchen. Spraying the toys with disinfectant is not sufficient for weekly cleaning and sanitizing.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • The water temperature in the following rooms is too cold for adequate handwashing:Navigator 1/2 shared bathroom - water temperature is 13CNavigator 3 bathroom - the automatic hand sink water temperature is 11CExplorer 2 shared bathroom - water temperature is 10CExplorer 4 bathroom the automatic hand sink is 11CThe water temperature in the following rooms is too hot and poses a scalding risk:Stargazer 1 - sink hot water temperature is 50.9CStargazer 2 - sink hot water temperature is 50.2C*Please adjust the water temperature or replace the faucets so the water temperature is between 35C to a maximum of 49C. Hot and cold running water must be provided at all sinks and warm water is necessary to facilitate handwashing.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • 1. The potty seats in Explorer 4 and Navigator 3 were not fully clean on the corners and underside. *Please ensure potty seats are regularly cleaned and sanitized thoroughly.2. In Navigator 3 the left toilet seat was loose.*Please tighten the toilet seat so it is safe and secure.

    • 08. Is this facility properly constructed (Non-Critical)?
      • Explorers 1 diaper change table has worn edges and is splitting at the back edge (portion abutting the wall). This condition prevents the change table from being effectively cleaned and disinfected. Centre Director has indicated the diaper change table is not required for this room and can be removed. Please remove or replace the change table.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Water temperature was measured in the rooms and washrooms:**Explorers 3 - 55C**Navigator 3 - 54C at room sink, 20C at washroom sink-Stargazer 1 - 54C-Stargazer 2 - 54C-Stargazer 3 - 45C-Explorers 1 - 17C at washroom sink, increased to 25C-Explorers 2 - 52C-Explorers 4 - 36C-Stellar 1 - 47C at room sink, 35C at washroom sink-Bathroom beside kitchen - 25CMaximum water temperature is 54C, with a recommendation of no more than 49C. Water temperature that is too cold will dissuade staff and children from adequately handwashing. Recommend water temperatures are maintained no less than 35C.*Please make adjustments to water temperature as needed.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • The bed linens in Stargazer 2 are stored in bins in a white storage cabinet. The blankets are overspilling the bins and are in contact with other bed linens. Please use bags or another method to prevent contact between bed linens while in storage. This was observed again December 5, 2024. Please adjust storage methods to prevent contact between bed linens.One bed sheet was left on a cot in the Explorer 4 room. Remove sheet and disinfect the adjacent cots. Do not stack cots until all bed linens have been removed.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Stargazer 1 and Stargazer 2 have one sink in the room used to wash hands after diapering as well as during meal times and play time.*Please develop a written decontamination plan to list the steps required to decontaminate the sink and area after diaper changes and prior to handwashing for meal times or play time.

    • 08. Is this facility properly constructed (Non-Critical)?
      • The edge of the change tables in Stargazer 1 and Stargazer 2 has the paint worn away and a slightly rough surface. Please either resurface this area so it is smooth, non-absorbent and easy to clean, or replace the change tables. The director indicated the change tables may be replaced with new ones shortly.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • The bed linens in Stargazer 2 are stored in bins in a white storage cabinet. The blankets are overspilling the bins and are in contact with other bed linens. Please use bags or another method to prevent contact between bed linens while in storage.
