- Operator onsite did not follow manufacturer's instructions for the intermediate level disinfectant (Barbicide). Operator does not have a measuring device for the dilution of the required amount of Barbicide and water. The label instructions state to mix 62.5 ml of Barbicide in 1 liter of water. Corrective Action:- Ensure you are following the manufacturer's instructions of this disinfectant.- Obtain a measuring device to measure the correct volume of Barbicide and water for mixing.

- Personal services notification (PSN) form was completed during the inspection. If you move locations or add/remove services in the future, please let this office know or complete a new PSN.

- Written procedures were completed during the inspection.Reprocessing sink will not be used for hand hygiene therefore written procedures for sink decontamination is not required.
