    • 06. Are proper protocols being followed in regards to client care?
      • Staff was observed diapering on the counter without the safety belt in the Toddler/preschool shared washroom. The Staff indicated the belt is not adjustable (too loose). The staff was told to put the mat and child on the floor and diaper on the floor only, until such time as a safe and functional belt is installed.
    • 23. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Critical)?
      • The three cribs in baby room each had two stacked mattresses in them. Crib must be used as per manufacturer instructions and no mattress to be more then 15cm in height.

      • Torn diaper change mat and therefore cannot be property disinfected- Toddler room
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Baby room counter was lifted/damaged: replace counter so that the area is watertight, smooth and easy to clean.
