      • Outstanding Violation (November 6, 2025)The wall in infant room 2 was observed to still be in disrepair.Please repair the wall the ensure it is smooth, easily cleanable, and impervious to contaminants.----------Initial ViolationVarious walls through the premises were damaged and/or in process of repair. This is an item that is continuously being monitored and resolved.- Ensure all walls, surfaces, etc. are smooth, durable and non-absorbent. For example:- Infant 2 room: wall by the window

      • 1. Toddler 4: Toys were stored by the diapering area. Please ensure toys are stored sanitarily in the toy rack. The diapering area should have items related to diapering.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1) The counter at the sink in Infant 2 was water damaged and deteriorated. The wooden substrate was rotted and needs to be replaced.2) Various walls through the premises were damaged and/or in process of repair. This is an item that is continuously being monitored and resolved.-ensure all walls, surfaces, etc. are smooth, durable and non-absorbent. For example:- Infant 2 room: wall by the window Toddler 3: Wall by the bulletin board

      • Preschool washroom: One of the soap dispensers in broken. Please replace.Toddler 2: The garbage bin has no foot pedal. Please ensure the garbage bins have foot pedal to dispose the paper towel after handwashing without touching the lid.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • The first aid kit is missing supplies such as antiseptic wipes. Please ensure the first aid kit is well supplied with necessary items.

      • Preschool 2: The area behind the heater is dusty. Please clean thoroughly.Toddler 4: The caulking behind the hand sink is in disrepair.

    • 06. Are proper protocols being followed in regards to client care?
      • Staff member didn't follow diapering procedures as they didn't use paper towel or liner before changing a child. -item was corrected during inspection and staff member added the paper towel.**Ensure the diapering procedure is followed each time for each child.The belt for diapering was under the change mat (Toddler 4).-ensure the belt is easily accessible and being used each time.

      • 1) The counter at the sink in Infant 2 was water damaged and deteriorated. The wooden substrate was rotted and needs to be replaced.2) The dryer vent was disconnected from the aluminum duct.-re-attached the dryer vent to the duct.3) Various walls through the premises were damaged and/or in process of repair. This is an item that is continuously being monitored and resolved.-ensure all walls, surfaces, etc are smooth, durable and non-absorbent.

      • 1. Preschool yellow room - curtains providing privacy in the toileting area are dirty.- Regularly clean and disinfect these curtains.

      • 1. Toddler yellow - blind broken.- Repair2. Some surfaces and toys in facility are beginning to show build up of grime/dirt. Some staff did not mention that cleaning is done prior to disinfecting with bleach.- Please ensure surfaces are cleaned with detergent/soap and water if they are dirty prior to disinfecting with bleach.3. Infant blue room - spray bottle nozzle for tables and toys was broken.- Repair.
