    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • (1) Some items that cannot be easily cleaned and disinfected had not been removed from play areas (included books, dress up clothes, plush toys, soft toys, etc).(2) There were no "DIRTY TOYS" bins in the rooms. During the inspection kids were observed mouthing toys and toys being put back into the play areas. Ensure all rooms have a designated "dirty toys" bin were mouthed toys can be placed into as soon as a child has finished playing with it. Ensure bin is easily accessible to staff so they do not have to leave the area to do remove toys from use until they can be cleaned and disinfected.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • (1) The sand table had not been removed from the Preschool room. It was full of sand, there were toys inside the table, and it had a cloth cover that could be easily removed. Ensure table is emptied out, cleaned and disinfected. (2) The sand table does not have a proper cover a blue cloth was being drapped over the table. Ensure a proper cover is available for the sand table.(3) Toys are being put straight into the dishwasher (sanitizer) without a washing and rinsing step first. Reviewed the proper toy cleaning procedures. The kitchen has a 2 compartment sinks that will be used for washing the toys in hot soapy water, rinsing in the second sink and then put into the dishwasher for the high temperature sanitizing step. For any toys that cannot be put into high temperatures, will drain one sink and use it to immerse toys in a fresh chlorine solution at 100 ppm for 2 minutes. Recommend purchasing a brush to scrub toys during the washing step in the first sink.

    • 05. Are communicable diseases at this facility being properly managed?
      • (1) Signs advising parents, visitors and staff of outbreak are not posted at the entrance. There are no handwashing signs posted for outbreak management. Ensure the signs are posted in a highly visible location as soon as someone enters the facillity. The daycare manager printed off the How to Use Alcohol Based Hand Rub poster and the STOP Clean Your Hands poster.
    • 06. Are proper protocols being followed in regards to client care?
      • (1) The bleach solution used for the diapering stations and all facility clean up are not at 1000 ppm as required during an outbreak. * A 1000 ppm bleach solution can be made by mixing 20 ml (4 teaspoons) of unscented household bleach with 1000 ml (4 cups) of water. To effectively kill viruses, a surface should remain wet with the bleach water solution for at least one minute. Food contact surfaces or toys require a rinse with clean water. (2) The chlorine test strips available on site had an expiration date of July 2022

      • (1) Alcohol based hand sanitizers on site were expired. Reminder these are meant for parents, visitors and staff. * During the outbreak, place a hand sanitizer at the entrance in a location out of reach of children and where adults can immediately sanitize their hands upon entering the facility.* Reminder: - Do not use hand sanitizers instead of handwashing. - Hand sanitizers do not work if hands are dirty. - Hand sanitizers are NOT acceptable during diapering or food preparation. - Hand sanitizers are not appropriate for infants and toddlers
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • (1) Gloves and masks used in the bathroom for diaper changing and cleanup are kept in the same bin as the diapers and other supplies. Have a separate storage area or containers for gloves and masks (all PPE).(2) Food grade (poly) gloves are being used for diapering and cleaning. These are meant for light-duty food prep tasks. They are not tear resistant. Obtain proper gloves.

      • Toddler room - a large lion stuffy was present in the room which cannot be effectively cleaned and disinfected. Please remove from use.Infant room - One of the small round cushions with butterflies on it was torn, exposing foam. Repair or removal required.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • A small slide of approximately 20 inches was present in the toddler room. Please provide protective surfacing under and around the slide.An example of this would be a double layer of interlocking foam mats.
