- Bleach solution bottles stored inside mirror cupboards in both the toddler and preschool washrooms. Chemicals stored in areas accessible to children and near hygiene facilities. Please ensure to secure the cupboards with safety locks or change its location to be out of reach.

- The bleach solution in the toddler room required for diaper mattress disinfection wasn't working. Ensure that the bottle is spraying enough water to ensure proper disinfection.No test strips in the facility to ensure the proper concentration.

- At 1:00 pm, the water table was observed to be unclean, with soap mixed into the water, posing a hygiene concern and increasing the risk of communicable disease transmission and potential ingestion hazards for younger children; therefore, the water in the water table must be replaced at least twice daily— ensuring that no soap or additives are mixed into the water, and all toys used in the water table must be properly washed, rinsed as needed, and thoroughly dried before each use to maintain safe and sanitary play conditions

- The facility uses an ammonia‑based disinfectant called “The Eliminator” to spray toys at the end of the day; however, the product container on site was found to be expired, and the disinfection process being used does not follow the manufacturer’s instructions, which require the solution to be rinsed off after application. Failure to rinse the disinfectant may pose a hazard to children, particularly those who mouth or handle toys, and may increase the risk of chemical exposure and related health concerns.

- Hot water measured 57-60°C throughout the facility.Water temperatures above 54°C pose a significant scald risk, especially to young children. (recommended at 49 degrees Celsius)Adjust hot water delivery systems to ensure temperatures at point‑of‑use taps do not exceed the recommended temperature. Verify temperatures after adjustment.

- There was a bottle of spray air freshener in the unlocked cabinet under the bathroom sink in the kinder room. Upon request, the bottle was disposed of. Please ensure any chemicals or other hazards are not accessible to children.

- The bleach solution in the senior toddler bathroom/diaper changing area was just soap and water and measured at 0 ppm chlorine. Upon request, the solution was remade and measured at 200 ppm chlorine. Please ensure all bleach solution bottles are prepared correctly.

- The kinder room bathroom did not have any paper towels. Upon request, a new supply was added to the bathroom. Please ensure each sink in the facility has paper towels nearby.

- - In the OSC room, the hot water baseboard radiant heating cover is not securely fastened to prevent access to the piping. Observed toys and other debris that is accumulating in the open space. Please ensure the cover is refastened to prevent child access to the piping and to prevent contamination to toys and other material that may become caught within the space.

- - Personal care items in the infant room are not being stored properly. The personal care products of one client are in contact with the personal care items of other clients. Please make sure that personal care products from different clients are kept separate.

- - In the toddler room adjacent to the kitchen, diapers are being stored on the washroom floor near the toilet. Please ensure the diaper supply is removed and stored in an area that minimizes the risk of contamination to the personal care products.

- - Bibs are being stored above the hand sink in the infant room for drying purposes. During handwashing, water splatter has the potential to reintroduce contaminants to the bibs. Please locate an alternate area for storing bibs during drying to minimize the potential for contamination.

- - In the toddler room, a diaper pad was observed resting against a toilet. Please ensure diapering pads are stored in areas that minimize the risk of contamination.
