      • Sterilization exposure time was not correctly recorded; instead, the cycle time was recorded.Corrective Actions:Going forward, ensure the exposure time for packaged cycle (8 mins) is documented, as that is the length time the chamber is maintained at 273F.

      • Sterility assurance documentation was not maintained with client records for pre-sterilized needles. Ensure batch number, lot number and expiry date are recorded with each client record.
    • 20. Is reusable equipment disinfected properly?
      • No high-level disinfectant was available for reprocessing of semi-critical tools.Obtain a high-level disinfectant to reprocess the semi-critical tools along with test strips to do daily testing prior to reuse of high-level disinfectant. and log the test results in the daily testing log sheet provided during inspection.
