- 1. Loop extractors were being disinfected with cavicide. Ensure these are disinfected with a high level disinfectant. 2. Bt micro facial tool cannot be disassembled and is being disinfected with cavicide. Discard tool. Ensure facial tools are disinfected with a high level disinfectant.

- 1. Botox is being reconstituted with preservative saline. Ensure a preservative saline is used for botox and botox like products as per the manufactures instructions (MIFU). 2. Botox is being reconstituted with preservative saline and saved in the fridge for more than 24 hours. Ensure reconstitute botox and botox like products are used within 24 hours as per the MIFU (product discarded). 3. Partial syringes are being used, using an aseptic technique, for Belotero filler and then stored in the cooler. These must be discarded once opened as per mifu. Do not save partial syringes. 4. Belotero filler is being stored in the cooler. This product must be stored at room temperature as per the MIFU.5. Nuceiva stored in at room temperature in a procedure room. This product must be stored between 2-8C.6. Nuceiva expired Jan 25 2026. Discarded onsite.

- The sterility information (lot number and the expiry date) of the pre-purchased sterile equipment (microneedling cartridges) is not being kept with that client's records. Please ensure the sterility information of the pre-purchased sterile equipment used on the client is documented with the client's forms.

- Cavi wipes that were used to sanitize surfaces were expired. Please replace.
