- 1. There was a gap on the bottom of the screen door for the back exit door. All entry points must be tightly fitted to prevent the entry of pests. Close the door or repair the screen door to ensure the gap is sealed to prevent the entry of pests.2. No pest control records were available. Pest control records must be readily available upon the request of a Public Health Inspector. Obtain the pest control records and retain onsite for review.

- ***Outstanding violation - noted on February 25, 2026*** Mice droppings were observed in the corner of the office area. January 16, 2025Mice droppings were noted in the dry storage room underneath the bulk floor bin.Remove the mice dropping. Clean and sanitize all areas contaminated by mice droppings.

There was significant grime noted on the exterior of several food equipment: - The exterior of the bulk containers - The exterior of the white standing freezer - The exterior of both clear double door coolers - The exterior of the microwaveFood equipment must be maintained in a clean and sanitary manner to prevent cross contamination of food. Thoroughly clean and sanitize the indicated areas.2. The bin used to store the pizza pans was in disrepair. Raw wood was exposed and dirt and debris were lodged in the crevices. The surface is no longer smooth durable and easy to clean. Repair or replace the bin and ensure the surface is smooth, durable and easy to clean.

- The scoop for the sugar was severely disrepaired. There were multiple cracks in the plastic scoop. Scoops must be maintained in good repair to prevent physical contamination of food. The operator discarded the scoop at the time of the inspection.

The facility must be maintained in a clean and organized manner to facilitate cleaning and monitoring of pests. Reorganize the indicated areas above and remove any items that are no longer in use.2. There was grime on the wall near the hand wash sink.All areas in a food establishment must be maintained in a clean and sanitary manner. Thoroughly clean the wall.
