      • A separate hand washing was not available. Hand washing will be done in the reprocessing sink. Decontamination of dual-purpose sink need to be completed. Reprocessing sink needs to be decontamination prior to reprocessing of equipment.

      • No high-level disinfectant was available in the facility do disinfect the semi-critical equipment.Semi-critical equipment must be disinfected with high-level disinfectant. Obtain a high level disinfectant.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Surfaces of the pillows were noted to be absorbent to the moisture. Covers of the pillows must be cleanable, wipeable and impervious to moisture.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • There was a toilet in the reprocessing area. Operator was advised to remove the toilet and cap the opening properly.
