- Child safety locks were not installed on the cupboards and refrigerators located under the counters in the Infant A and Infant B rooms.Please install child safety locks.

- The child safety clips on a highchair located in the infant room was observed to be broken, effecting its ability to effectively secure the child.Please repair the clip or replace the entire highchair.

- The first aid kit located in the infant room were missing tweezers, CPR masks, and alcohol swabs. Please ensure all first aid kids are fully equipped with alcohol swabs within their expiry date, tweezers and CPR masks.

- 1) Cloth couches throughout the facility were observed without removable, launderable covers.Please ensure the couches are equipped with a cover that can be removed for regular cleaning.2) The filter of the air purifier located in the Preschool Room was observed to be covered in dust.Please replace the filter.3) Chipped paint was observed on the diaper changing station, exposing raw wood.Please refinish or replace the diaper changing station to ensure it is smooth, easily cleanable, and impervious to contaminants.4) Play mats in the Pre K 3b room were observed to be peeling and in disrepair, making them difficult to clean and sanitize.Please remove and replace the play mats with ones that have smooth, impervious, and easily cleanable surfaces.

- Infant room, 4yrs old room: Cubies were not secured. - Secure to prevent falling onto children. 4yrs old room: Electrical cords were hanging from a shelf. - Secure to prevent entanglement hazard. 3A room: An electrical outlet was detaching.- It was blocked immediately. - RepairInfant room: Cupboards under the sinks are only taped, they do not have child safety locks.- Provide child proof locks.

- No safety straps at 2 change tables (PB and IB) - Provide safety traps to prevent accidents. No liner was used under children during diapering in PB and P3A. - Provide liner under children during diapering.
