- Raw fish stored above cut onions and above cooked potatoes in the walk-in cooler. This configuration introduces the posibility of cross contamination. The PHIs asked that these vegetables be moved to a different location. This was corrected during the inspection.Please ensure that raw protein is not stored above cut vegetables or cooked foods at any time.

- 1. Multiple stored food items and food containers were found in the coolers without a lid or cover to protect the food from potential contamination.Please ensure that all food items in storage are covered at all times. 2. Handleless scoops were found in multiple bulk storage bins in the dry storage area. Please ensure that all scoops have a handle that does not touch the bulk foods or have the scoops/measuring cups stored outside of the bulk food container. 3. A large bowl of cooked rice was observed to be cooling in the kitchen prep sink. The sink was actively dripping water into the adjacent compartment.Please ensure the prep sink is not used to store any cooked food at any time.

- Plumbing issues noted for the prep sink. The dishwashing sink also had a small leak. Please ensure the plumbing issues are repaired, remediated or replaced.

- Additional cleaning is required in these areas: *The oven in the kitchen * The can opener and handle of the long-handled machine on the table that housed the can opener. * The commercial stand mixer located at the back of the facility. *Door handles, underneath wall mounted shelving units and high touch surfaces. *Walls/floors/Ceiling all showed the build-up of visible debris or soot. * The storage bins on the top of the lids and on the sides/lip of the bulk food storage bins. Please ensure all food are stored in a clean and sanitary manner and protected from contamination at all times. * The ice machine had the buildup of visible debris on the outside and the inside of the machine. Please clean and sanitize the ice machine.Please ensure that these noted areas are thoroughly cleaned and sanitized.

- Walk in cooler - observed a container of raw fish being stored above a pail of cauliflower rice and sauce.

- 10. Are food temperatures during storage and processing being controlled to maximize food safety?
- Improper cooling of rice in place. Rice was being cooled with the lids on, which slows the process of cooling. Please keep foods uncovered when cooling and ensure they are cooled down within 2h at room temperature before being placed into the cooler.
- 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
- Prep cooler with meat was measuring at around 9-15C. Internal temperature of meat was at ~9C. Owner was asked to try to keep the cooler door closed for an hour and if the temperature doesn't drop, to move all high risk foods into another cooler until that cooler is repaired.

- Build-up of dust and staining observed on ceiling tiles near ventilation canopy. Owner indicated they will clean and paint the tiles the same day.
