      • 1. Turn off ice machine and melt all ice in the machine. Do not use ice in machine as it is potentially contaminated. Clean and sanitize ice machine as per approved sanitation plan to remove mold/mildew-like film. Facilitate frequent cleaning and sanitation to prevent build-up in future.

All designated hand washing stations must be adequately supplied with hot and cold running water, liquid soap and paper towel to ensure staff are washing their hands to prevent contamination of food.

Do not keep food in this cooler until it has been repaired and can consistently keep food at 4 deg C or less.       • Observation: Observation: Severe amount of rodent droppngs observed through out the kitchen area, specifically behind laundry bins by back door, in front of electricity panels, behind beer tanks by chest freezer, underneath food prep. tables, grills, coolers, and freezer, and on the lowest part of cashiers counter. Previous pest report (Apr.9) indicated heavy feeding activities in all baiting stations (exterior), but no activity found indoor.

      • Observation (CORRECTED DURING INSPECTION): Two line coolers operating at 50-58F and food temperatures ranged from 50-62F. All potentially hazardous foods MUST be kept at 40F unless a time tracking system has been approved and is in place and food is discarded after 2 hours of being taken out of cooler.
      • Corrective Action(s): All potentially hazardous foods were discarded during inspection. Repair person was called during inspection and is due to arrive tomorrow morning. For tonights dinner service put inserts on ice and change ice before it all melts and put only the amount of product in inserts that can be used in less than 2hours.

      • Observation (CORRECTED DURING INSPECTION): Main bar glass washer had no detectable sanitizer at final rinse. Both main and upstairs bar glass washer sanitizer bottles were very low with remaining chlorine sanitizer almost depleted of active ingredient.
      • Corrective Action(s): Monitor these on a daily basis and replace bottles as needed. Recommend covering air gap around the sanitizer line and mouth of sanitizer bottle to slow down the evaporation of chlorine from the bottle.

      • Observation: 1) Three fly strips containing dichlorvos (white casing with yellow pad on inside) found throughout the kitchen storage areas. Product label reviewed with staff during inspection.
      • 2) Ice wand stored in freezer directly on shelf.
      • Corrective Action(s): 1) Remove these fly strips as they are not allowed for use in these areas as per the manufacture's instructions. Due: today

      • Observation: Observed three dead mice in snap traps and droppings along floor-wall join in kitchen storage areas. Staff stated they have already called in a pest control company who has coming in weekly for the past four weeks and that the issue has significantly improved since the pest control company started coming. Pest control reports available on site for inspection.
      • Corrective Action(s): Send copies of the last four weeks of pest control reports to undersigned inspector for review by tomorrow. Clean away all old droppings as well as dead mice in traps. If not already doing so, follow all recommendations given by pest control company.

      • 2) Multiple items (such as old construction supplies, equipment no longer in use, old signage, etc) stored throughout premise storage rooms on the floor.

    • 205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
      • Observation (CORRECTED DURING INSPECTION): (1) Two main line cooler temperatures ranged from 11.2C to 15.2C, required temperature is 4C or less. Temperature logs have not been maintained.
      • (2) Garlic butter made with fresh garlic found out at room temperature, staff stated that it had been out at room temperature since 11am (3.5 hours ago). Fresh garlic in/under oil creates a significant risk of botulism
      • Corrective Action(s): (1) Cooler contents sorted through during inspection and all potentially hazardous foods were voluntarily discarded. Have cooler adjusted/repaired and do not use this unit for refrigeration until it is repaired and can consistently maintain 4C (40F) or less. In the mean time, ice baths may be used for any short term food storage in this area but ensure only enough food for the next meal rush is brought out and stored in this area and if there are any leftovers after the rush in the ice bath, they must be discarded. If these two coolers continue to have difficulty maintaining proper temperature, a longer term option may be to use time stamps and discard product after two hour "expiry" has past - consult with undersigned inspector before implementing a system like this.
      • (2) Garlic butter found out at room temperature was voluntarily discarded. If fresh garlic is used in/under oil it can only stay out at room temperature for two hours and then be discarded. Alternatively acidified garlic or powdered garlic may be used and can be stored at room temperature.

      • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): Bar glass washer had no detectable chlorine sanitizer after being run. Bottle of sanitizer appeared faint in colour indicating that the concentration of the active ingredient in the bottle may have significantly diminished. When unit was primed, bubbles in line were observed going up and down indicating a possible problem with air getting into the line.
      • Corrective Action(s): Adjust/repair unit so that it consistently sanitizes with 50ppm chlorine or another approved sanitizer. Until this unit is repaired, all glasses must be washed in the back dishwasher.
