      • No quat test strips available.- Obtain test strips.

      • Paper towels were not provided in the dispenser/hand's free mechanism for both handwashing sinks. - Provide paper towels in hand's free manner Soap was running low at the prep area sink. - Equip sufficient soap. The hot water tap at the prep area handwashing sink was shut off/disconnected.- Hot water must be supplied. A faucet handle of the prep sink was missing.- Equip handleThe handwashing sink near the kitchen was blocked by a laundry basket.- Ensure free of access to handwashing sink at all time.

There are food debris and grease accumulated at the soup pot and behind the soup pot, and behind the cookline. - Ensure the areas are cleaned and sanitized.

      • Wall beside/behind the dishwasher is water damaged and is no longer cleanable. Repair the wall. Ensure that the wall is smooth, cleanable and impervious to moisture.

      • Thick accumulation of dust was noted around the ceiling vents and on the walls. Ensure that the air ventilation system is working properly. Thoroughly clean this area.
