- Observation: Live rat observed in the mop/janitorial room.
- Rat droppings observed under the wire rack in the hallway
- Rodent droppings noted in some staff workstations floors and behind the mini chest freezer next to the front walk-in cooler
- Potential rodent entry hole noted on the right side of the upright mixer in the back storage area beside the back walk-in cooler. Hole was noted near wires entering walls.
- Corrective Action(s): Have pest control conduct an evaluation for holes in the building.
- Have pest control contracted for service minimum every 2 weeks.
- Provide pest control technician's service report to area health inspector within 2 days of receipt. Provide reports for a minimum of 6 service occasions.
- Fill all rodent entry holes identified
- Follow all pest control technician's recommendations in a timely manner
