      • Observation: 1 dead rat found in snap trap in the storage room beside the mop sink grease trap
      • Mice droppings noted behind kitchen area mini chest freezer
      • Rat droppings noted in corners of some workstations
      • Corrective Action(s): Clean the above noted areas, this must be done on a more frequent basis.

      • 306 - Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Empty station next to the entrance door was found to have leftover equipment (camping chairs and carts), and molding food in the double upright cooler
      • Corrective Action(s): Conduct a deep clean of the empty work station. Due 1 week

      • Observation: Live rat observed in the mop/janitorial room.
      • Rat droppings observed under the wire rack in the hallway
      • Rodent droppings noted in some staff workstations floors and behind the mini chest freezer next to the front walk-in cooler
      • Potential rodent entry hole noted on the right side of the upright mixer in the back storage area beside the back walk-in cooler. Hole was noted near wires entering walls.
      • Corrective Action(s): Have pest control conduct an evaluation for holes in the building.
      • Have pest control contracted for service minimum every 2 weeks.
      • Provide pest control technician's service report to area health inspector within 2 days of receipt. Provide reports for a minimum of 6 service occasions.
      • Fill all rodent entry holes identified
      • Follow all pest control technician's recommendations in a timely manner

      • 306 - Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: Grease build up observed on and around the exhaust fan on the west facing wall of the kitchen as well as around the walls behind the altosham and deep fryers.
      • Brown liquid stains observed on the walls of the walk-in cooler
      • Corrective Action(s): Clean the above noted areas, ensure that this is done on a more frequent basis.
      • Violation Score: 3
      • 309 - Chemicals, cleansers, & similar agents stored or labeled improperly [s. 27]
      • Observation: Rodent poison noted in trays in the hallway of the facility. Rodent poison is not permitted in food service facilities
      • Corrective Action(s): Remove the rodent poison immediately. Snap and glue traps only
