      • In the walk-in freezer, food items stored in metal trays were covered with black plastic bags. The bags did not appear to be food-grade.- Operator is to remove plastic bags and ensure the plastic bags used for food storage are verified as food-grade prior to use.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • Lighting in the walk-in freezer was dim and far corner of freezer was difficult to inspect without a flashlight. - Please either increase the intensity of the current fixture or install an additional light fixture(s).

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • 3 buckets of QUAT solution was tested >400ppm. Dispenser was tested and was tested at 200ppm with both facility and inspectors test strips.- Kitchen manager took the 3 QUAT solutions and diluted them and retested at 200ppm.- Kitchen manager was advised to monitor the dispenser and check to ensure solution is dispensing consistently at 200ppm.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • Iodine test strips for bar gas cleaner expired in 2021.- Acquire new test strips.

    • 03. Is all food in this facility fit for human consumption and not unwholesome, spoiled, or adulterated?
      • There were food items found during the inspection that were not in good condition (moldy oranges). The food items were discarded during the inspection. The inspector informed the operator that a system must be in placed that monitors the condition of all food items, and that they must be discarded immediately and not be served or used as an ingredient.An air-tight packaged steak was found inflated during the inspection. The inspector informed the operator that inflation (swelling) can occur in packaging due to gases formed as a result of spoilage taking place (bacterial growth/metabolic activity). The steak was discarded during the inspection. Thus, moving forward, please ensure to discard all inflated steak packages.
    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • There were cleaning cloths stored on food-contact surfaces. The inspector informed the staff/operator that cleaning cloths must either be single use or be fully submerged in a sanitizer bucket solution. The staff/operator agreed to do the latter. Please ensure to have all cleaning cloths be fully submerged in a sanitizing bucket solution at all times after use.
    • 07. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Critical)?
      • Food items (chicken breasts) in the cooler were found not adequately covered. The inspector informed the staff that all food items must be covered to prevent cross-contamination of the food during storage. Please ensure to cover all food items.
    • 18. Are general nuisance conditions, other than those noted in other violations, being prevented at this food facility (Non-Critical)?
      • Portable ventilation fans were being used above the food preparation area. The fans were noted as dusty during the inspection. The inspector instructed the operator to remove the fans.
