- The surface of a chopping board was chipped off.-The chopping board was discarded.-Ensure that the chopping boards are smooth so that they can be easily cleaned and sanitized.

- 1) There was coffee beans spilled in the drawer used for storing coffee.2) Drawers containing large utensils and tongs were dirty.-Please clean the areas.

- No chlorine test strips to test bleach sanitizer solution. PHI-trainee gave operator some test strips, and instructed operator to purchase test strips to ensure proper the bleach sanitizer solution is at the proper concentration of 100ppm chlorine.
