- Backpacks in the playground were placed right next to each other touching. Please ensure appropriate separation of personal belongings can be achieved, such as by placing the backpacks in baskets or hanging them on hooks spaced at least 12" apart.

- One of the toilets upstairs was not flushing. A plumber was called. Please ensure the toilet is fixed.

- 1. The upstairs playroom had an unstable black toy shelf with a broken top shelf. Please remove the shelf, or repair/replace it in a way that ensures it cannot fall over.2. Bingo dabbers were part of the craft materials for children's use. The manufacturer's instructions for use say to keep out of reach of children, therefore these are not suitable for use. Please ensure they are not used by children.

- The test strips available to test the concentration of chlorine disinfectant were expired in 2022 and 2023. Please ensure new test strips are purchased immediately.
- 08. Is this facility properly constructed (Non-Critical)?
- 1. The drywall and paint was chipped in several areas, including the playroom upstairs under the cubbies, and the windowsill area in the baby room. Please ensure holes are patched properly and painted so they can be cleaned and disinfected appropriately. 2. There was tape or residual glue from tape on the floor, walls, countertops, and shelves. Please ensure all tape and residual glue is removed completely so the areas can be fully cleaned and disinfected.
- 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
- 1. The rug in the upstairs playroom was curling up in one corner, which poses a tripping hazard. Please rotate the rug so that the corner is held in place or remove the rug.2. There were some boxes and equipment at the foot of the staircase that blocks part of the emergency egress. Please ensure these items are moved and that nothing is stored here in the future.
- 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
- 1. Several things have been repaired using duct tape, including the baseboards in the main floor baby room and the back stairwell, the window on the second-floor playroom, the bottom of the staircase, and some of the toy bins. The tape has started to deteriorate and is no longer able to be cleaned and disinfected properly. Please ensure the tape is removed fully, including the glue residual, and repair the areas in a manner that renders them smooth, non-absorbent and easy to clean.2. The general cleanliness of the facility was lacking. The light switches were dirty, the toy baskets had residue on them, the carpets had dirt and stains, the floor under the carpets was gritty, the comfy area in the toddler room with the blankets had sand under the mats, the stairwell railing was dirty, the high shelf in the washroom had dust on it, and the upstairs personal cubbies had crumbs and dirt at the bottom of them. Please ensure all these areas are cleaned thoroughly. Devise a deep cleaning schedule to ensure areas are cleaned regularly and nothing is missed.

Ensure that the hand sinks have adequate temperature for adequate handwashing (temperature at hand sinks are recommended to be mid 30'sC)
