- 1. The ice machine gasket was in disrepair. Please repair or replace the ice machine. - Corrected. Photos were received through email showing that the gasket in good repair. 2. The handle for the front prep cooler was missing. Please install a replacement.

- No evidence of vermin was observed.Monthly pest control records were not available for review. Please complete monthly pest control records and have them available onsite for review. The operator later provided photos of the completed pest control checklist.
