      • Push pins and staples were observed on the wall in the Junior Toddler Room. Please remove all push pins, as they may become dislodged and pose a hazard to children. Consider using safer alternatives such as tape, sticky tack, or Velcro. If staples are used, they should be covered with tape.Electrical outlets were observed without safety covers in the Baby Room. Ensure all electrical outlets are fitted with appropriate safety covers to prevent access by children.

      • The diaper changing mat in the Senior Toddler Room was observed to be ripped and cracked. Replace the mat, or if designed for reversible use, flip it to the intact side to ensure a smooth, non-absorbent, and easily cleanable surface.

      • -Chemical and knife drawers in the kitchen were not kept locked. This was corrected at the time of inspection. The kitchen door should be kept closed and chemical/knife drawers locked.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -There was a purse with a long strap in the Senior Toddler Room; this is a strangulation hazard. Please remove or remove the strap.
    • 06. Are proper protocols being followed in regards to client care?
      • -There was only one sink in the Baby Room and the sink is used for diapering. At the time of inspection food was stored on the counter all around the sink and toys were stored on the diaper table. The change table and sink area is considered a contaminated space. Ensure that the space is kept clear at all times or relocate diapering to another location.-The area under the diapering mat in the Baby Room was unclean and was being used to store items. Please ensure the diapering mat and the space underneath is kept clean and cleaned/disinfected after each use.
    • 23. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Critical)?
      • -Bedding was stored on cots, rather than folded in the middle of each cot. Please ensure that bedding is stored separate for each child; folded in the middle of each cot, in a labelled bag, in cubbies etc.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • - Staff items were accessible to children in the Senior Toddler Room. Staff items should be stored in an area that cannot be accessed by children.
